新生儿疼痛表现以及干预方法的研究

摘　　要：目的:观察新生儿对疼痛的反应,探讨非营养性吸吮干预对新生儿疼痛的干预效果。方法:收集60例新生儿临床资料,随机分为对照组和干预组各30例,干预组在疼痛时采用非营养性吸吮干预,观察两组新生儿疼痛的表现,并于疼痛前、疼痛后1 min、5 min分别记录心率(Heart rate,HR)、呼吸(Respiratory rate,RR)、血压(Blood pressure,BP),采用新生儿面部编码系统(Neonatal facial coding system,NFCS)进行疼痛评估。结果:新生儿对疼痛反应主要表现在面部表情变化,以皱眉、张口、挤眼最为常见,另外还有肢体动作以及呻吟、尖叫、甚至屏气等;同时还导致HR、RR、BP的较大波动。而在疼痛时给予非营养性吸吮干预能缓解新生儿的疼痛,两组在疼痛后1 min、5 min的疼痛程度以及HR、RR、BP变化差别有极显著性意义。结论:新生儿对疼痛极为敏感,反应较强烈,而进行有效的干预能缓解新生儿的疼痛。Objective：To observe the reactions of neonates to pain,explore the effect of nonnutritive sucking on neonatal pain.Methods：60 neonates were divided into intervention group and control group randomly,nonnutritive sucking was used in intervention group after pain stimulation.The clinical manifestations and heart rate（HR）,respiratory rate（RR） and blood pressure（BP） were recorded before and 1 minute,5 minutes after pain stimulation,respectively.Neonatal facial coding system（NFCS） was used to evaluate the degree of neonatal pain.Results：The reactions of neonatal pain manifested mainly in the facial expressions,including frowning,gaping,squeezing eyes,and the changes of HR,RR and BP.Other reactions such as limbs actions,groaning,squealing,breathholding were also observed.There was significant difference in the score of NFCS and the changes of HR,RR and BP between intervention group and control group.Conclusion：The neonates are sensitive to pain stimulation with strong reactions,which maybe released by effective interventions.
