摘 要:目的研究新生儿窒息后多器官功能障碍综合征(multipleorgandysfunctionsyndrome,MODS)的发生及其与病情预后的关系。方法窒息后24h之内入院的足月新生儿89例,进行经皮血氧饱和度和心电监测3d,同时作肝功、肾功、心肌酶、电解质、血常规、尿常规、血气分析及心电图和头颅CT等检查,结合临床表现统计窒息后全身炎症反应综合征(systemic inflammatory respons syndrome,SIRS)和MODS的发生情况及其疾病的转归情况。结果与轻度窒息组相比,重度窒息组危重病评分、SIRS和MODS发生率以及多器官损伤的程度明显增加,差异有统计学意义(P<0.05);与预后好组相比,预后差组SIRS和MODS发生率明显增高(P<0.05)。结论新生儿窒息可导致SIRS和MODS的发生,与窒息程度和疾病的预后密切相关。Objective To investigate the incidence and prognosis of multiple organ dysfunction syndrome(MODS) in asphyxiated neonate. Methods 89 asphyxiated infants who admitted hospital within 24 hours after their birth were enrolled in this study, we monitored all neonates'transcutaneous oxygen saturation and electrocardiogram for 3 days, and also tested their hepatic functions, renal functions myocardial enzymes, electrolytes, blood counts , urinelysis,blood gos analysis, electrocardiogram and CT scans. At last, we assessed the incidence and the prognosis of SIRS and MODS in those asphyxiated infants. Results Compared with mild asphyxia groups, the score for neonatal critical degree,the total incidence of S RS/MODS, and their critical degree of MODS in severe asphyxia groups were increased( P 〈 0.05) ;In another words,the incidence of SIRS/MODS in bad prognosis groups are very high compared with good prognosis groups( P 〈 0.05). Conclusion The incidence of SIRS and MODS are very high in asphyxiated infant, it may be related to the prognosis and critical degree of asphyxiated infant.
