摘  要:目的调查匡氏琼脂PNB培养基用于痰分枝杆菌直接鉴定的效果。方法采用匡氏琼脂PNB培养基直接鉴定痰标本中的结核菌和非结核分枝杆菌;并采用琼脂PNB培养基和多重PCR技术对分离物重鉴定。结果 281例痰标本,110例培养阳性,经匡氏PNB培养基直接鉴定,非结核分枝杆菌感染3例(3/110,2.7%),结核菌和非结核分枝杆菌混合感染7例(7/110,6.4%),结核菌感染100例(100/110,90.9%)。匡氏琼脂PNB培养基痰菌直接鉴定法对结核菌和非结核菌感染病例的灵敏度和特异性达100%。结论匡氏琼脂PNB培养基痰菌直接鉴定法,能快速准确鉴定非结核菌感染和混合感染,操作简易,适合推广应用。Objective To evaluate the value in typing myeobacteria directly in sputum using Kuang's agar PNB medium. Methods Kuang's agar PNB medium was adopted for differentiation of Mycobacterium tuberculosis complex ( M. TB) and nontuberculous mycobacteria(NTM) directly from sputum samples. Mycobacterium isolates from sputum specimens were identified by Kuaug's agar PNB medium and multiprimer PCR. Results 110 of 281 sputum samples were positive . Three (3/110,2.7%) of these were infected by NTM ;7 (7/110,6.4% ) were infected by both M. TB and NTM ; 100 (100/110,90.9% )were infected by M. TB. The sensitivity and specificity of differential identification of M. TB and NTM by Kuang's agar PNB medium were 100%. The cases infected with NTM could be detected in 3 - 14 d by Kuang's agar PNB medium, Conclusions Direct differentiation of M. TB and NTM from sputum samples by Kuang's agar PNB medium offered a rapid and simple technique for the patients infected with NTM or with both NTM and M. TB , and its application in clinical laboratory is easy.
