压力负荷诱导的大鼠舒张性心力衰竭模型的建立

摘　　要：目的建立高血压导致的舒张性心力衰竭的动物模型。方法Wistar大鼠50只随机分为肾上腹主动脉缩窄组(n=18)、肾下腹主动脉缩窄组(n=16)、假手术组(n=16),假手术组作为对照组。术后采用超声心动图检查测定大鼠心脏结构和功能,术后12周测定血流动力学指标,进行病理组织形态学分析。结果术后4周肾上和肾下组大鼠分别出现室间隔厚度(1.83±0.11、1.74±0.12)、左室后壁厚度(1.81±0.11、1.67±0.13)、左室心肌重量(0.89±0.16、0.81±0.12)比对照组(分别为1.38±0.05、1.34±0.06、0.63±0.13)增加(P<0.05),其它指标均无统计学差异(P>0.05);术后12周肾上腹主动脉缩窄组出现心衰表现,等容舒张期时间(50.39±9.69)比对照组(39.71±17.96)延长(P<0.05),左室舒张末压(28.83±3.47)比对照组(5.15±2.68)升高(P<0.01),-dp/dtmax(4712.81±1888.44)比对照组(7132.18±1428.08)降低(P<0.01),Em/Am下降(P<0.05),射血分数和+dp/dtmax无显著变化(P>0.05)。结论肾上腹主动脉缩窄组出现了心力衰竭的临床表现,左室舒张功能受损但射血分数保持正常,符合人类高血压导致的射血分数正常性心力衰竭的特点。Objective To establish the rat model of heart failure with preserved ejection fraction resulting from hypertension. Methods 50 6-week-old Wistar rats were randomly assigned to three groups：Suprarenal abdominal aorta coaretation group （n=18） ,Infrarenal abdominal aorta coaretation group（n=16）and the sham-operated animals were taken as control group（n=16）,The clinical manifestation of heart failure were followed up carefully after operation. We measured the changes of cardiac structure and function using echocardiography. The hemodynamic parameters were determined before sacrifice. Results In operated group, marked ventricular hypertrophy was observed 4 weeks after operation. In suprarenal abdominal aorta coarctation group and infrarenal abdominal aorta coarctation group IVSD was 1.83±0.11,1.74±0.12;LVPWD was 1.81±0.11,1.67±0.13 and LVmass was 0.89±0.16, 0.81 ±0.12 increased （P〈0.05）, The control group IVSD was 1.38±0.05 ,LVPWD was 1.34±0.06,LVmass was 0.63±0.13. In snprarenal abdominal aorta coarctation group 12 weeks after operation,the clinical manifestation of heart failure presented,and 1VRT 50.39±9.69 than control group 39.71 ± 17.96 extended （P〈0.05） and LVEDP 28.83 ±3.47 than control group 5.15 ±2.68 inc teased （ P〈0.01 ）, and - dp/dtmax 4712.81±1888.44 than control group 7132.18±1428.08 decreased （P〈0.01）. Tissue doppler image indicated that e/a ratio of annulus of mitral valve decreased（P〈0.05）. However,EF and ＋dp/dtmax were preserved. Conclusions The operated group 1 an- imals presented the clinical manifestation of heart failure, and diastolic function of left ventricle injured, however, ejection fraction was preserved, which corresponded to character of heart failure with preserved ejection fraction resulting from hypertension.
