摘 要:目的:探讨CT脑灌注成像技术在颈动脉狭窄致脑缺血性疾病患者外科治疗术前评估中的临床应用价值。方法:对21例颈动脉狭窄患者(19例单侧狭窄,2例双侧狭窄)行多排螺旋CT灌注成像,针对其脑血流量(CBF)、对比剂平均通过时间(MTT)进行计算分析,评价患者脑血流动力学变化情况。结果:19例患者的双侧脑灌注在术前有显著差异,CBF降低(rCBF=0.874±0.037),MTT延长(rMTT=1.320±0.072)。结论:CT脑灌注在颈动脉狭窄患者的术前评估脑血流动力学异常变化方面具有较高的临床应用价值,术前发现脑灌注明显降低的病例,外科治疗对其症状改善有意义。Purpose: To explore the clinical value of multi - slice CT perfusion imaging on cerebral hemodynamics of carotid artery stenosis causing cerebral ischemia and on pre - surgical management evaluation of carotid artery stenosis. Methods: Twenty - one patients with carotid artery stenosis were enrolled in this study, including 19 unilateral stenosis and 2 bilateral stenosis. Multi - slice CT perfusion was used to measure the cerebral blood flow(CBF), mean transit time(MTT) . The patients' cerebral hemody- namics was evaluated. Results: Nineteen of 21 patients had impairments ipsilateral to the stenosis side, including decrease of CBF and delay of MTT .Conclusion: CT perfusion can sensitively evaluate the cerebral hemodynamic impairment in patients with moderate or severe carotid artery stenosis, and therefore if CT perfusion show disorder in cerebral hemodynamic, surgical management can improve the symptom.
