冠心病患者Lorenz散点图的特点与临床价值

摘　　要：目的探讨冠心病患者Lorenz散点图的特点及其临床意义。方法冠心病患者50例（A组）源于2007-01～2008—02在哈尔滨医科大学附属第一医院心血管内科住院的患者，男28例，女22例，平均年龄（59．6±10．7）岁；正常组48例（B组）源于同一时间在我院体检中心体检的健康人群，男34例，女14例，平均年龄（56．2±8．1）岁。入院或入选后佩戴12导联Hoher（24h）．由专业人员对自动输出的Lorenz散点图图形进行判断。结果冠心病患者的Lorenz散点图图形改变与以往类似，如呈梭状或棒状，亦有少部分呈现不定状；而正常人则多数呈彗星状；非彗星状图形诊断冠心病的敏感度为84％，特异度为75％，阳性似然比和阴性似然比分别为3．36和0．21。结论散点图在冠心病患者的诊断中有一定的临床价值，在心电图正常时有特别意义。Objective To study the characteristics and clinical significance of Lorenz plots in the patients with coronary heart disease （ CHD ）. Method 50 patients of CHD （ 28 males, 22 females） from the First Affiliated Hospital of Harbin Medical University and 48 healthy volunteers （ 34 males, 14 females） were selected. Each of them had been worn Holter ECG monitoring for twenty -four hours, then the Lorenz plots result was analysed by professional. Results The figure of Lorenz plots indicated that fusiform, rodshaped or a small number of indefinite shapes were displayed in the patients of CHD and comet - like shape in the majority of healthy volunteers. The sensitivity and specificity of CHD determined by non - comet - like shape was 84% and 75% respectively. The positive and negative Likelihood Ratio was 3.36 and 0.21. Conclusion Lorenz plots is significant in clinical diagnosis of CHD, especially for the patients with normal ECG.
