摘  要:目的探讨血清载脂蛋白A1(APOA1)、白蛋白(ALB)及C-反应蛋白(CRP)三种急性时相反应蛋白(APRP)在急性百草枯中毒(APP)中的变化及意义。方法据入院时服毒量及临床体征分轻度中毒组及中、重中毒组两组共35例,于第3天采集空腹静脉血3mL,应用OLYMPUSAU2700全自动生化分析仪免疫比浊法检测血清APOA1、CRP,终点法检测血清ALB,并选28例门诊健康体检者做对照。结果APP患者血清APOA1(1.230±0.317)g/L、ALB(37.628±5.364)g/L较对照组(1.392±0.125)及(41.343±2.373)g/L分别有显著性降低,差异有统计学意义(P〈0.05);CRP(17.82±18.47)mg/L较对照组(2.86±3.72)mg/L极显著性升高(P〈0.01),且中、重度中毒组(27.39±20.34)mg/L较轻度中毒组(9.02±3.15)mg/L升高明显(P〈0.01)。结论APP时,患者血清APOA1、ALB及CRP改变机体是应激、全身炎症反应综合征(SIRS)及多脏器损伤等的综合表现,观测上述三种APRP的变化,对了解机体的损害状况,观察病情,指导临床治疗有重要作用。Objective To discuss the changes and clinical significances of three acute -phase reaction protein ( APRP), serum apolipoproteinA1 ( APOA1 ), albumin ( ALB ) and C - reactive protein (CRP) in acute paraquat poisoning (APP)patients. Methods 35 patients were divided into two groups: the mild group and moderate or severe group according to the dosage of taking poison and clinical signs on admission. 2 days after admission, 3 mL empty stomach venous blood was collected and serum APOA1, ALB and CRP were detected by immunoturbidimetry. 28 healthy people were selected as control group. Result Serum APOA1 ( 1. 230 ± 0.317 ) g/L and ALB ( 37.628 ± 5. 364 ) g/L of APP patients was significantly lower than those of control group [ ( 1. 392 ± 0. 125 ) g/L and (41. 343 ± 2. 373 ) g/L, P 〈 0. 05 ]. CRP level (17.82 ± 18.47mg/L} of APP patients was significantly higher than that of control group [(2. 86± 3.72) mg/L, P 〈0.01 ]. The level of CRP in moderate or severe group (27.39 ±20.34)mg/L was significantly higher than that of mild group ( 19.02 ±3.15 ) rag/L, P 〈 0. 01 ]. Conclusion The change of serum APOA1, ALB and CRP of APP patients was the comprehensive performance of stress, systemic inflammatory response syndrome (SIRS), multiple organs damage and so on. The detection of three APRP helps us understand the impaired condition of body, observe patient's condition and guide clinical treatment.
