自制鼻饲匀浆液在基层医院鼻饲患者中的应用体会

摘　　要：目的探讨自制鼻饲匀浆液在基层医院鼻饲患者中的应用效果。方法将68例鼻饲患者随机分为观察组和对照组,每组各34例。两组均经留置胃管定时鼻饲,其中观察组采用自制匀浆液,对照组采用力衡匀营养膳,分别于鼻饲前及鼻饲后第21d监测两组患者营养状况。比较两组患者鼻饲前后营养状况、鼻饲并发症发生情况及患者家属对两种营养液的接受情况。结果与鼻饲前比较,两组患者采用两种模式鼻饲21d后,其营养指标均有所提高,均P<0.05;但两组鼻饲前后营养状况组间比较,差异无统计学意义(均P>0.05)。两组患者鼻饲并发症比较,观察组腹胀腹泻发生率低于对照组(P<0.05)。家属对营养液的接受情况比较,观察组高于对照组(P<0.05)。结论自制鼻饲匀浆液取材方便、配方合理,胃肠道反应少、价格低廉,易受农村患者及家属欢迎。Objective To investigate the effect of the self-prepared homogenate solution for nasal feeding at primary-level hospitals. Methods 68 cases undergoing nasal feeding were randomly divided into observation group and control group equally. The former took self-prepared homogenate solution for nasal feeding and the latter took the nutritional meals regularly via the indwelled nasogastric tube. The nutritional status of all the patients in the two groups were monitored respectively before and 21 d after nasal feeding. The complications from nasal feeding and the attitude of their families to the two kinds of nutritious solutions were compared between the two groups. Results The nutrition indexes of the two groups were significantly improved after 21-day nasal feeding （P 〈 0.05）. However, there were no significant differences in comparison of before and after nasal feeding in each group （P 〉 0.05）. For nasal complications, the incidences of abdominal distention and diarrhea in the observation group were higher than those in the control group （P 〈 0.05） and for attitudes of their families, the observation group tended to accept the solution more than the control group did （P 〈 0.05）. Conclusion Self-prepared homogenate solution is characterized by its convenience in obtaining materials, its reasonable and sanitary preparation, its few gastrointestinal reactions and above all its inexpensiveness, which all contribute its popularity among rural patients.
