摘 要:目的:探讨2型糖尿病眼底增殖改变与冠状动脉病变的关系。方法:对330例2型糖尿病合并冠心病患者进行眼底检测及冠状动脉造影,根据眼底是否具有增殖性改变进行对比分析,并应用多因素分析方法探讨与眼底增殖改变相关的危险因素。结果:2型糖尿病伴眼底增殖改变者,年龄较大,病程较长,尿微量白蛋白阳性检出率增加,TG、TC及HbA1c显著性升高;多支血管病变及钙化病变受累比例增高;Logistic回归分析显示,经调整糖尿病病程、HbA1c、微量白蛋白尿后,2型糖尿病眼底增殖改变仍与冠状动脉钙化相关。结论:2型糖尿病眼底增殖改变与冠状动脉钙化相关。Objective: To study the relationships between coronary artery lesion and proliferative retinopathy in type 2 diabetic patients with coronary heart disease. Methods: The study was carried out by the analysis of the data from coronary arteriography and fluorescence fundus angiography in 330 type 2 diabetic patients with coronary heart disease. There was a comparative analysis between the patients with proliferative diabetic retinopathy (PDR) and the patients without PDR. Logistic regression analysis was applied to explore various factors to PDR. Results: Diabetic patients with PDR had longer average course, older age and higher incidences of microalbuminurine than those with nonproliferative diabetic retinopathy (NPDR). Patients with PDR also had higher values of TG, TC and HbA 1 c than those with NPDR. It was more possible for individuals with PDR to be involved with coronary artery multivessels or coronary artery calcium (CAC). Logistic regression analysis showed that PDR was related to the average course of diabetic patients, HbA 1 c, the higher incidences of micro-albuminurine and CAC. PDR was significantly associated with CAC, even after adjustment of other risk factors. Conclusions: PDR was associated with CAC in diabetic patients.
