深圳市龙岗区基层预防保健体系运行机制和效果分析

摘　　要：目的:研究下放街道管理对深圳龙岗区基层疾病预防控制和卫生监督工作的影响,为完善基层预防保健体系管理和运行机制提出政策性建议。方法:以问卷调查和深度访谈相结合的方法对深圳龙岗区12家街道卫生监督分所(预防保健所)的运行情况进行研究。相关资料进行统计学分析。结果:深圳市龙岗区街道卫生监督分所(预防保健所)人力资源总体素质水平偏低,基础硬件配置落后,经费来源主要靠业务收入,疾病预防保健工作有所加强,卫生监督监测工作有所减弱。结论:下放街道管理,弱化卫生管理职能,不利于区域卫生统筹;非卫生专业人员进入公共卫生队伍,非卫生专业额外任务增多,影响卫生监督(预防保健)工作正常开展。建议理顺基层预防保健体系管理和运行机制,实行全行业管理,坚持政府主导原则,加大公共卫生投入,加强公共卫生人才队伍建设。Objective In order to propose policy mechanism of the primary prevention and healthcare primary prevention and public hygiene surveillance suggestion on perfecting the administrative and operational system, we try to find out the impact on the performance of in Longgang District of Shenzhen when we decentralize the authority to street level. Method This research is based on a questionnaire survey and in--depth interview that was conducted to 12 branch offices of Health Surveillance in Longgang District of Shenzhen, investigating the current status and performance of these branches. Result The quality of the staff working in these offices is quite low and some fundamental equipment is outdated. Moreover, the business income is the major financial support of these offices. Much emphasis has been put on disease prevention but the duty of surveillance is set back. Conclusion The decentralization of authority weakens the capacity of health management and goes against the overall planning of health system. In addition, involving non--professional work force and the increasing health--unrelated tasks lead to the ineffectiveness of performance. We suggest that it is necessary to clarify the administrative and operational mechanism of primary healthcare and prevention, implement the industry- wide management based on the government--led principle ,increase the public health investment and enhance the quality of human resource.
