主动固定电极在右室流出道间隔部起搏中的应用(附15例报告)

摘  要:目的评价主动固定螺旋电极在右室流出道间隔部起搏应用中的可行性和安全性。方法选择15例有心脏起搏适应证的患者采用主动固定电极行右室流出道间隔部起搏,观察电极置入时间、X线曝光时间、心电图QRS波宽度及起搏阈值、感知、阻抗等,术后1、3个月随访测定起搏阈值、感知、阻抗,观察有无电极脱位及相关并发症。结果记录置入时间为(16.33±4.56)min,X线曝光时间为(11.2±4.5)min,起搏阈值为(0.82±0.19)V,感知为(12.68±3.05)mV,阻抗为(990±0.146)Ω,起搏的QRS波时限为(0.15±0.03)s。随访起搏阈值、感知、阻抗稳定,未见电极脱位等并发症。结论主动固定螺旋电极在右室流出道间隔部起搏中的应用是可行和稳定的。Objective To evaluate the feasibility and safety of active fixed spiral electrode in the right ventricular outflow tract pacing. Methods Fifteen patients with cardiac pacing indication performed right ventricular outflow tract septal pacing used active fixed electrode. The time of procedure and X-ray exposure, the width of QRS wave, threshold, perception, impedance of the pacing were noticed and followed-up 1, 3 months after operation to observe the changes of above parameters, electrode location and possible complications. Results Procedure time was ( 16. 33 ± 4. 56) min, and X-ray exposure time was ( 11.2 ± 4. 5 ) min. The related pacing parameters were : pacing threshold (0. 82 ±0. 19) V, perception ( 12. 68 ± 3.05 ) mV, impedance (990 ±0. 146 ) Ω, pacing QRS wave duration (0. 15± 0. 03) s. In 3 months followed-up after operation, pacing parameters were stability, no electrode dislocation and other complications. Conclusions Application of active fixed spiral electrode in the right ventricular outflow tract septal pacing is feasible and stable.
