摘  要:目的 研究趋化因子受体4(CXCR4)在鼻咽癌细胞中的表达,间质细胞衍生因子1α(SDF-1α)在鼻咽癌远处靶器官中的表达,探讨CXCR4和(或)SDF-1α在鼻咽癌器官特异性转移中的作用。方法应用逆转录聚合酶链反应(RT—PCR)和免疫组织化学法分析30例鼻咽癌、15例正常鼻咽组织中CXCR4mRNA和蛋白的表达及其同临床病理学因素之间的相关性,应用免疫组织化学法分析鼻咽癌患者的正常颈部淋巴结(包括颈深上和颈深下淋巴结)、骨髓、肺、肝脏和肾脏、结肠(各5例)中SDF-1α蛋白的表达。结果RT—PCR检测结果显示,鼻咽癌组织中CXCR4mRNA相对表达强度(0.71±0.22)显著高于正常鼻咽组织(0.14±0.07;F=27.94,P〈0.05);免疫组织化学检测结果显示,鼻咽癌组织中CXCR4蛋白的表达(1.58±0.59)显著高于正常鼻咽组织(0.51±0.22;F=17.75,P〈0.05)。鼻咽癌组织中CXCR4mRNA和蛋白的表达与临床分期、淋巴结转移、细胞分化程度显著相关(均P〈0.05)。SDF-1α蛋白在鼻咽癌患者的颈深上淋巴结、骨髓、肺、肝脏中表达较高(2.35±0.67),而在颈深下淋巴结、肾脏和结肠中表达较弱(0.68±0.23),差异有统计学意义(t=10.13,P〈0.01)。结论CXCR4的表达与鼻咽癌的转移密切相关,CXCR4和(或)SDF-1α在鼻咽癌器官特异性转移中可能发挥重要作用。Objective To explore the role of CXCR4/SDF-lα axis in organ-specific metastasis of nasopharyngeal carcinoma (NPC) by assessment of CXCR4 expression in NPC cells and SDF-lα expression in distant target organs of NPC. Methods Thirty patients with NPC and fifteen normal subjects were recruited in this study. The expressions of CXCR4 in NPC and normal cases were identified by RT-PCR and immunohistochemistry (IHC), then the relationship between CXCR4 expression and clinicopathological factors was analyzed. IHC was also used to analyze the SDF-lα protein expression in normal cervical lymph nodes (including normal superior and inferior deep cervical lymph nodes ), bone marrow, lung, liver, kidney and colon tissues of NPC patients (5 cases/each group). Results The relative expression level of CXCR4 mRNA in NPC (0.71 ±0.22) was significantly higher than that of normal nasopharynx tissues (0.14 ± 0.07, F = 27.94, P 〈 0.05 ). The relative expression level of CXCR4 protein in NPC ( 1.58 ± 0.59) was significantly higher than that of normal nasopharynx tissues (0.51±0.22, F = 17.75, P 〈 0. 05 ). The high expression levels of CXCR4 mRNA and protein in NPC were closely related to clinical stage, cervical lymph node metastasis and cancer cell differentiation (P 〈 0.05). SDF- 1α protein was strongly expressed in normal superior deep cervical lymph nodes, bone marrow, lung and liver (2.35 ± 0.67) , but absent or very poor expression in inferior deep cervical lymph nodes, kidney and colon tissues (0.68 ± 0.23 ), and the differences between them were statistically significant (t = 10. 13, P 〈 0.01 ). Conclusion CXCR4 is closely correlated to metastasis of nasopharyngeal carcinoma. CXCR4/SDF-1 α axis may play an important role in organ-specific metastasis of NPC.
