摘  要:目的探讨胃癌全胃切除术后早期肠内营养和肠外营养的应用效果。方法46例全胃切除患者随机分为两组,23例术后行肠外营养治疗(PN组),23例术后早期行肠内营养治疗(EN组)。比较术前、术后第1、5、9天总蛋白、白蛋白、转铁蛋白,术后第9天氮平衡,肠道功能恢复时间,术前及术后1、5、9天CD3、CD4、CD8阳性细胞百分比、CD4/CD8比值及CRP。结果EN组与PN组在改善患者营养状况方面无显著性差异(P>0.05)。EN组能明显促进患者肠道功能恢复,提高机体免疫水平(P<0.05)。结论胃癌全胃切除术后早期肠内营养比肠外营养更具优势。Objective To investigate the efficacy of parenteral nutrition and early enteral nutrition in patients with gastric carcinoma after total gastrectomy. Methods 46 patients after total gastrectomy were randomly divided into two groups,23 patients supplied with parenteral nutrition(PN group), 23 patients supplied with early enteral nutrition(EN group). The change of total protein, albumin and transferring before operation and postoperative day 1,5 and 9,nitrogen balance before operation and postoperative day 9,bowel movement restoring time and the percentage of positive CD3,CD4,CDs eell,CD4/CD8 ratio and CRP before operation and postoperative day 1,5 and 9 were compared. Results There was no significant difference between two groups in improving state of patient's nutrition(P〉 0.05),but early enteral nutrition could improve gastrointestinal function and immune function(P〈 0.05). Conclusion Early enteral nutrition is more efficient than parenteral nutrition after total gastreetomy.
