摘  要:目的探讨乳腺导管声学造影对乳头溢液性病变诊断的临床价值。方法48例乳头溢液疾病术前行高频超声、乳管声学造影检查及乳管镜检查,对比分析超声检查结果与术后病理结果。结果病理结果表明36例导管内占位中32例为良性肿块,4例恶性肿块;其余12例均为乳腺导管扩张症。常规超声可显示其中20例病变,而导管声学造影可显示其中25例占位。3例导管扩张症在常规超声出现导管内低回声肿块,注入造影剂后显示其中2例为分泌物沉积所致。结论乳腺导管声学造影可提高导管内小病变的检出率,显示详细的病变部位及扩张的导管,高频超声与导管声学造影是诊断乳腺导管疾患有价值的检查方法。Objective To compare the diagnostic values of sonographic galactography and sonographic galactography on localizing abnormalities of the discharging ducts in patients with spontaneous nipple discharge. Methods All the 48 cases were examined by high- frenquent ultrasound , sonographic galactograph and fiberoptic duetoscopy before operation. The results of three examinations were compared with that of post- operation pathology diagnosis. Results According to the final diagnosis which reported by pathologist, there were benign neoplasms in 32 cases , malignant in 4 cases and ductal ectasia in 12 cases. An echogenic intraductal tumor was detected in 20 cases by sonography and in 25 cases by sonographic galactography among 36 patients with tumors. 3 cases of ductal ectasia were found hypoechogenic intraductal mass which was compose of ductal secretion, and 2 cases of them were discriminatd with sonographic galactography. The abnormal ducts were surgically removed after examination. Conclusion Sonographic galactography improves the detection rate of tiny lesions in mammary duct. High- frequency ultrasound and sonographic galactograph are feasible means for diagnosis of the mammary duct disease.
