组织速度成像对左心室长轴方向收缩后收缩现象的研究

摘　　要：目的应用定量组织速度成像(QTVI)评价左心室长轴方向局部心肌收缩后收缩(PSS)的特点并探讨其临床意义。方法在组织成像条件下对心肌梗死(MI)、扩张性心肌病(DCM)、肥厚性心肌病(HCM)及高血压病(HBP)患者的左心室纵向各节段的收缩后收缩进行测定,并与正常组对比分析。结果①PSS发生率:HBP组、HCM组、DCM组、MI组PSS阳性率明显高于正常对照组(P<0.05),并呈依次逐渐增高改变;②PSS分布规律:HBP组、HCM组、DCM组的PSS主要分布在前后间隔及下壁,与正常组相似,而MI组的分布则无明显规律性;③PSS持续时间:4组疾病组的PSS持续时间均明显长于正常组,二者比较差异有统计学意义(P<0.001),其中以MI组持续时间最长;④PSS峰值速度:4组疾病组的PSS峰值速度均高于正常组(P<0.05),其中以MI组峰速最高。结论左心室纵向PSS能反映心肌功能情况,其PSS的发生率越高、持续时间越长、峰值速度越大提示心肌功能损害的可能越大。Objective To assess the features of postsystolic shortening（PSS） of left ventricle longitudinal regional myocardium and probe its clinical meanings by quantitation tissue velocity imaging （QTVI）. Methods Left ventricle regional myocardial PSS of MI, DCM, HCM and HBP patients were examined and compared with that of normal group by QTVI. Results （1）The positive rates of HBP group, HCM group, DCM group which increased gradually were obviously higher than that of normal group（P〈0.05） ;（2）The PSS of HBP group, HCM group and DCM group was mainly distributed over anterior, posterior septal and inferior wall, which were similar to normal group. There was no obvious distribution regularity in MI group;（3）PSS duration of four disease groups was much longer than that of normal group （PSS duration in MI group was the longest one）, and there was significant difference among them（P〈0. 001） ;（4）PSS peak velocity of four disease groups was much higher than normal group（PSS peak velocity in MI group was the highest one） and there was significant difference among them（P〈0.05）. Conclusion Left ventricle longitudinal PSS can reflect myocardial function. It suggests that the higher of PSS incidence, the longer of PSS duration and the bigger of PSS peak velocity, the more serious of myocardial damage.
