曲美他嗪对不稳定性心绞痛患者血管内皮功能的保护作用

摘　　要：目的:观察曲美他嗪对不稳定性劳力型心绞痛患者血管内皮功能的保护作用,并探讨其可能原因。方法:61例不稳定性劳力型心绞痛患者,被随机分为常规治疗组(30例)和曲美他嗪组(31例),常规治疗组给予扩冠、抗凝、调血脂等治疗,曲美他嗪组在常规治疗基础上加用曲美他嗪20mg,3次/d,服用8周。结果:血浆hs-CRP:治疗前曲美他嗪组和常规治疗组均较健康对照组升高;治疗后曲美他嗪组与常规治疗组均显著降低,曲美他嗪组较常规治疗组降低显著(P<0.05)。治疗前FMD:曲美他嗪组和常规治疗组均较健康对照组明显降低;治疗后曲美他嗪组与常规治疗组均显著升高,曲美他嗪组较常规治疗组升高更加显著(P<0.05)。结论:曲美他嗪在不稳定心绞痛患者治疗中能减轻血管炎症反应,改善血管内皮功能的作用。Objective：To observe protection effect of trimetazidine therapy on vascular endothelial function in patients with unstable angina pectoris （UAP） , and study its mechanism. Method：The 61UAP patients were randomly divided into routine therapy group and trimetasidine group. The routine therapy group was received routine therapy fpr UAP. The trimetazidine 20mg, 3 times daily were given to each cases of trimetazidine group besides routine therapy. The plasma highsensitive C -reactive protein （hs -CRP） and brachial artery blood flow mediated endotheliumdependent dilation（FMD） were detect before and 8 weeks after treatment. Its results were compared with health control group. Results ： Befor therapy the plasma hs - CRP routine therapy group and trimetazidine group significantly increased compared with health control group[ （5.46 ± 2.33）mg,/L, （5.12 ± 2.48 } mg/L： （1.16 ± 0.65） mg/L; after therapy？ plasma hs - CRP of 2 therapy groups are both decreased [ （ 3.32 ± 1.05 ） rag/L, （4.07 ± 1.27 ） mg/L , P 〈 0. 01 ], and trimetazidine group decreased very significantly（ P 〈0.05 ） ; Before therapy？ brachial artery FMD？ of routine therapy group and trimetazidine group significantly decreased compared with？ health control group[ （9.25 ± 5.77 ） %, （ 9.16 ± 4.53 ） % ： （ 13.50 ± 6.45 ） %, P 〈 0.05 - 〈 0. 01 ], after therapy FMD of 2 routine therapy groups both increased significantly（ [ （ 13.89 ± 3.51 ） mg/L, （ 11.64 ± 4.41 ） mg,/L , P 〈 0.05 - 〈 0.01 ], and trimetazidine group increased very significantly than that of routine therapy grou （ P 〈 0.05 ）. Conclusion ： Trimetazidin can decrease inflammation reaction, and improve vascular endothelial function.
