摘 要:目的通过检测造影剂相关性肾病患者尿液中β2微球蛋白(β2-MG)、N-乙酰-β-D-氨基葡萄糖苷酶(NAG)及肾损伤分子1(KIM-1)的水平,了解β2-MG、NAG及KIM-1在造影剂相关性肾病患者中的改变及其与肾功能之间的关系。方法收入49例造影剂相关性肾病患者,同时收入52例健康志愿者,以ELISA法分别检测其尿液中的β2-MG、NAG及KIM-1水平,分析其与患者肾功能之间的关系。结果造影剂相关性肾病患者尿液中β2-MG、NAG、KIM-1分别为(594.62±87.93)mg/L、(24.52±4.64)U/L、(4.68±0.83)ng/mL,均高于健康对照组[β2-MG(108.16±10.55)mg/L,NAG(8.74±1.75)U/L,KIM-1(0.28±0.10)ng/mL],差异有统计学意义(P均<0.05);患者组的肌酐清除率Ccr为(25.09±7.50)ml/min,低于健康对照组的(83.75±33.84)ml/min,差异亦有统计学意义(P<0.05)。结论造影剂相关性肾病患者尿液中β2-MG、NAG及KIM-1水平较健康对照组明显升高,反映了肾小管的损伤情况,可作为造影剂相关性肾病患者肾功能的检测指标。Objective To explore the relationship between the changes of urinary β2-microglobin (β2-MG), N-acetyl-beta-D-glucosaminidase (NAG) and kidney injury molecule-1 (KIM-1) in patients with contrast agentinduced nephropathy (CIN) by analyzing the level of urinary β2-MG, NAG and KIM-1. Method 49 cases of CIN patients and 52 cases of healthy volunteers were enrolled into the study. The levels of urinary β2-MG, NAG and KIM-1 were determined by ELISA. Result The level of β2-MG (594.62±87.93)mg/L, NAG (24.52±4.64)U/L and KIM-1 (4.68± .83)ng/mL in CIN patients was significant higher than the healthy subjects [β2-MG (108.16± 10.55)mg/L,NAG (8.74±1.75)U/L,KIM-1 (0.28±0.10)ng/mL]. And the level of creatine of CIN patients (25.09±7.50)ml/min was significant lower than the healthy control subjects (83.75±33.84) ml/min. Conclusion The level of urinary β2-MG, NAG and KIM-1 is elevated in CIN patients, indicating the impairment of tubular function. β2-MG, NAG and KIM-1 may be useful indicators for the monitoring of kidney function in CIN patients.
