摘 要:目的:探讨球囊撑开纳米人工骨注入微创跟骨增高的技术方法与治疗效果。方法:2005-02/2009-03大连大学修复重建外科研究所与全国多家医院合作完成该项目。入选平足患者62例124足,全部为女性,年龄18~52岁,平均28.5岁。身高145~165cm,平均身高154.1cm。所有患者足跟无创伤、破溃感染和皮肤疾病,长时间负重行走有疼痛等不适感。采用纳米磷酸钙胶原基人工骨材料,行球囊撑开纳米人工骨注入微创跟骨增高术。术后定期随访,X射线摄片,观察纳米人工骨成骨的进展情况,纳米骨是否有移位、变形,患者在负重行走情况下是否有疼痛感,以及切口是否有红肿渗出等感染症状。结果:所有患者创口均完全愈合,无明显瘢痕,无感染破溃等并发症。术后当天复查X射线片,人工骨材料降解不明显。随着时间的延长,人工骨材料逐渐降解。3个月后X射线片显示自体跟骨组织与人工材料融合,显影清晰,3年后X射线片显影清晰,自体跟骨组织与人工材料完全融合,无移位及压扁。增高幅度为2.0~4.2cm,平均增高3.0cm,所有病例平足不适感消失,长时间负重行走无疼痛感。结论:球囊撑开纳米人工骨注入微创跟骨增高的技术定位准确、疗效确切、创伤小、恢复快,是安全有效的跟骨微创增高技术,同时能够有效地改善平足畸形。OBJECTIVE: To explore the technological mechanism and therapeutic effect of balloon nano-artificial bone infused into the calcaneus on the correction of height. METHODS: This study was performed in the Institute of Reparative and Reconstructive Surgery, Dalian University and some other hospitals. A total of 62 female patients (124 feet) were admitted, aged 18-52 years (average 28.5 years). Height was from 145 cm to 165 cm (average height 154.1 cm). No trauma, broken, infection and skin disease occurred in feet of all cases. Flat feet were found in all cases, uncomfortable feeling occurred after long time walking. Balloon nano-hydroxyapatite/collagen bone was infused into heels for making calcaneus heightening. Following-up was regular, and X-ray was taken to observe the osteoplastic progress, displacement and deformation of nano-bone, weight-loading walking, and infections. RESULTS: Excellent healing occurred in all cases, no dominant scar was found, and no complications such as infection and broken happened. X-ray on the operation day showed that no dominant degradation happened. Artificial graft was degraded with the time going on. Three months later, autograft and allograft were fused, and image was clear. The domain of heightening was from 2.0 cm to 4.2 cm, average 3.0 cm. Uncomfortable and painful feeling after long time walking was disappeared. CONCLUSION: Balloon infusion with nano-hydroxyapatite/collagen bone is safe, accurate operation position, little invasion, rapid recovery, so it is an ideal method for heightening using the calcaneus microinvasive technique. At the same time, it can effectively improve the malformation of flat foot.
