血清sFas、TNF-α、IL-6水平对风湿性瓣膜病致慢性心力衰竭心功能评价作用的研究

摘　　要：目的研究血清sFas、TNF-α、IL-6水平对风湿性二尖瓣病变致慢性心力衰竭(CHF)患者手术前后心功能评价作用。方法实验组为27例风湿性二尖瓣病变致CHF患者,二尖瓣置换术(MVR)术前、术后3个月分别采集血清;20例同期健康体检者作对照组;ELISA法测定血清sFas及TNF-α和IL-6水平,分析其与手术前后心功能关系。结果术后sFas、TNF-α、IL-6水平下调但仍显著高于对照组,房颤及中重度三尖瓣返流(TR)组sFas水平显著高于窦律及轻度TR组,sFas水平与心功能程度相关。结论血清sFas水平很好反映了MVR前后心功能变化,对评价CHF的病情及预后具备潜在前景。Objective To explore the evaluation of serum level of soluble Fas （sFas）, tumour necrosis factor-α （TNF-α）, interleukin-6 （IL-6） on heart function perioperatively of chronic heart failure （CHF） results from rheumatic mitral lesion. Methods 27 patients of CHF results from rheumatic mitral lesion as experimental group, and 20 health people as control group. The blood samples were collected before and 3 months after mitral valve replacement （MVR） and serum levels of sFas TNF-α, IL-6 were measured by ELISA. Results The level of serum sFas, TNF-α, IL-6 were significantly reduced after the operation. The level of serum sFas with atrial fibrillation or severe tricuspid regurgitation （TR） was significantly higher than that with sinus rate or mild TR and that was closely related to NYHA class. Conclusion The level of serum sFas reflect the change well of heart function perioperatively, and that had potential perspective on evaluating the degree and prognosis of CHF.
