摘　　要：目的:检测肺心病患者M2受体的自身抗体并探讨M2受体自身抗体在肺心病发病中的作用及其与右心结构的关系。方法:用SA-ELISA法检测110例肺心病病人和100名健康人血清中抗M2受体的自身抗体。结果:肺心病组病人血清M2受体的自身抗体的阳性率及平均几何滴度[48.18%,80(1.846±0.351)]均高于对照组[13%,40(1.602±0.369)](P<0.05)。肺心病组中M2受体的自身抗体阴性组、阳性组比较表现为:血氧分压(PaO2)呈逐渐降低趋势,二氧化碳分压(PaCO2)、右室流出道内径、右室内径呈逐渐增高趋势P<0.05)。结论:M2受体的自身抗体可能与肺心病的发病过程密切相关,M2受体的自身抗体可能与肺心病发病过程中的心脏损害特别是右心扩大有关。Objective：To investigate whether antibody against M2 - Muscarinic acetylcholine Receptors are related to chronic pulmonary heart disease. Methods ：Two group are chronic pulmonary heart disease group（ n = 110） and normal control group（ n = 100）. Results： The synthetic peptide M2 -muscarinic acetylcholine receptor which corresponded to amino acids sequences of the second extracellular loop of human were used as antigens by an SA - ELISA to screen sera from two groups. The rate of positive of antibody against M2 - Muscarinic acetylcholine Receptors in the sera of chronic pulmonary heart disease group （48.18%） were significant higher than those of normal control group（13%） .The geometric means of the anti - M2 - receptor antibody titers in chronic pulmonary heart disease group and normal control group were 1 ： 80 and 1 ： 40, respectively. Conclusion ： In chronic pulmonary heart disease group, the rate of positive antibody and the titers of antibodies to M2 - muscarinic receptor in the sera were significant higher than normal control group and normal control group. The patients suffered from chronic cor pulmonale with positive sera of anti - M2 receptor autoantibodies have shown significant different with negative sera of anti - M2 receptor autoantibodies in Doppler color echocardiogram examinations, but in the artery gases analyses. Thus, it may shows antibodies to M2 - muscarinic receptor play a very important role in the development of the pathological in chronic pulmonary heart disease.
