摘  要:目的探讨不同氧流量对大鼠Ⅲ期及以上压力性溃疡创面愈合的影响,为临床压疮局部氧疗提供依据。方法将30只大鼠制作Ⅲ期及以上压疮模型,将成模的24只大鼠随机分为M组、L组、H组各8只,M组局部未作处理;L组及H组局部予以氧疗,氧流量分别为3~5L/min、6~10L/min。每天1次,每次20min,连续4d。肉眼观察并记录各组创面的大小、渗出液情况;每次氧疗后检测局部组织血流量;于治疗第5天检测血管内皮生长因子(VEGF)含量。结果氧疗组大鼠压力性溃疡创面渗出液减少,创面较干燥,且压疮部位肿胀减轻;创面直径较M组显著缩小(P〈0.01),尤其是H组;VEGF阳性细胞数、局部组织血流量显著高于M组(均P〈0.01)。结论局部氧疗可通过增加VEGF表达,促进压疮创面细胞增殖,增加毛细血管数量,从而改善血液循环,促进压疮愈合。尤以高流量氧疗治疗效果明显。Objective To investigate the effect and mechanism of local oxygen treatment on healing of decubitus. Methods Animal model of decubitus was made in 24 SD rats and they were divided into three groups of M,L and H randomly. The animals in group M were treated only by the way of replacing dressing, and those in group L and group H by using 3-5 L/min and 6-10 L/min oxygen flow rates respectively on the wound surfaces of decubitus besides routinely replacing dressing. During the course of experiment, the size of wound surfaces and the volume as well as the color of exudate in decubitus were observed before and 72 h after treatment; blood flow in local wound surface tissues at the first three days of treatment were tested respectively. On the 5th day, the contents of vascular endothelial growth factor(VEGF) were histologically observed. Results The size of wound surfaces and the volume of exudate in group L and gruop H were significantly smaller and less than group M at the same time, with the above indexes in group H being the most significant. There was significant difference in blood flow in local wound surface tissues among 3 groups after treatment (P〈0.01 for both). Immunohistochemisty analysis also showed an increase of VEGF positive cell in group H (P〈0.01). Conclusion Applying high flow rates of oxygen on local wound surface could facilitate healing of decubitus. One of the mechanism may be through increasing the expression levels of VEGF.
