人乳头瘤病毒、人巨细胞病毒和单纯疱疹病毒Ⅱ型感染与慢性前列腺炎的关系

摘　　要：目的:探讨正常男性和慢性前列腺炎患者前列腺内的人乳头瘤病毒、人巨细胞病毒和单纯疱疹病毒Ⅱ型的分布。方法:未婚无性经历男性20例和慢性前列腺炎124例,其中Ⅱ型前列腺炎(慢性细菌性前列腺炎)13例,Ⅲ型前列腺炎(慢性非细菌性前列腺炎)111例(ⅢA 58,ⅢB 53)。前列腺液取材后行HPV(总型)、CMV、HSV-2的PCR检测。结果:正常人群病原学检测均阴性;Ⅱ型前列腺炎的HPV、CMV和HSV-2分离率分别为7.7%(1/13)、15.4%(2/13)、23.1%(3/13)。Ⅲ型前列腺炎中HPV、CMV和HSV-2分离率分别为12.6%(14/111),20.7%(23/111),13.5%(15/111)。Ⅲ型前列腺炎中,HPV和CMV分离率与前列腺液白细胞计数呈正相关(P<0.05)。结论:慢性前列腺炎患者前列腺内有更高的CMV、HPV、HSV-2分离率,推测前列腺内病毒可能是慢性前列腺炎的感染性病原微生物。Objective ：To investigate the virus of Human papillomavirus （ HPV）, cytomegalovirus （CMV） and herpes simplex virus （HSV） -2 in the prostate of patients with chronic prostatitis and normal young male adults. Methods： 124 patients with chronic prostatitis and 20 normal young male adults were included in the study, among which 13 cases were classified into typeⅡ prostatitis, 111 cases were classified into typeⅢ prostatitis. Expressed prostatic secretion （EPS） samples obtained were subjected to urological evaluation including routine microscopic examination of EPS, bacteria culture. Arbitary PCR were performed to confirm viruses including HPV, HSV -2 and CMV. Results：In normal control group, all results from etiologic detection were negative. Isolating rate of HPV, CMV and HSV-2 in prostatitis of typeⅡ were 7.7% （1/13）, 15.4% （2/13） and 23.1% （3/13）, respectively. Among chronic type Ⅲ prostatitis, isolating rate of HPV, CMV and HSV- 2 was 12. 6% （14/111）, 20.7% （23/111 ） and 13.5% （ 15/111 ）, respectively. Positive correlation was found between white cell count in EPS and infection rate of HPV or CMV. Conclusions： Our results indicate that various genital viruses exist in the prostate of patient with chronic prostatitis. Probably, CMV, HPV, HSV -2 may be included in pathologic process of chronic prostatitis.
