慢性非细菌性前列腺炎患者前列腺液TNF-α、IL-8的检测及临床意义

摘　　要：目的:探讨前列腺按摩液(EPS)中细胞因子白细胞介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)在Ⅲ型前列腺炎[慢性非细菌性前列腺炎/慢性盆底疼痛综合征(CAP/CPPS)]诊断、分型中的意义。方法:ELISA法检测50例临床诊断为Ⅲ型前列腺炎患者EPS中IL-8及TNF-α水平。结果:慢性非细菌性前列腺炎EPS中ⅢA型组TNF-α水平与ⅢB型组、对照组比较,有显著性差异(P<0.05);ⅢB型组TNF-α水平与对照组比较,无显著性差异(P>0.05)。ⅢA型组IL-8水平与ⅢB型组、对照组比较,有非常显著性差异(P<0.01);ⅢB型组与对照组比较,有非常显著性差异(P<0.01)。所有患者EPS中TNF-α水平与WBC计数无明显相关性(r=-0.167,P>0.05),IL-8水平与WBC计数的相关性分析显示呈正相关(r=0.836,P<0.01)。结论:检测Ⅲ型前列腺炎患者EPS中IL-8及TNF-α水平可能有助于Ⅲ型前列腺炎的分型诊断,并可作为了解病情和评价治疗效果的一个有价值的指标。Objective： To investigate the significance of interleukin-8 （IL-8） and tumor necrosis factor-α （TNF-α） in expressed prostatic secretion （EPS） for diagnosis and classification of prostatitis m [chronic nonbacterial prostatitis/chronic pelvic pain syndrome （CPPS）]. Methods： IL-8 and TNF-α in EPS of 50 patients with prostatitis Ⅲwere detected by ELISA. Results： The level of TNF-α in EPS of patients with chronic nonbacterial prostatitis groupⅢ A was significantly different with group m B and the control group （P〈0.05）. Level of TNF-α had no statistical significance between group IU B and the control group （P〉0.05）. Level of IL-8 in group IU A had very significant difference compared with group IU B and the control group （P〈0.01）; compared with the control group, IL-8 in group ⅢB had a very significant difference （P〈0.01）. TNF-α level in EPS of all patients had no significant correlation with WBC count （r=-0.167, P〉0.05）, IL-8 level and WBC count showed a positive correlation by the correlation analysis （r=0.836, P〈0.01）. Conclusion： The detection of IL-8 and TNF-α levels in patients with prostatitis may contribute to diagnosis and classification of prostatitis Ⅲ as the valuable indicators to understand the state of an illness and evaluate the effect of treatment.
