尿流动力学检查在前列腺增生患者术前的临床意义

摘　　要：目的:评价尿流动力学检查在前列腺增生症(BPH)患者术前诊治中的意义。方法:对72例BPH患者术前进行尿流动力学检测;根据检测结果选择不同方法进行治疗,其中,经尿道前列腺电切(TURP)56例,行前列腺切除术8例,膀胱造瘘术5例,保守治疗3例。结果:72例中有57例存在膀胱出口梗阻,10例可疑梗阻,5例无梗阻。其中,逼尿肌收缩力减弱13例,逼尿肌功能不稳定35例,12例伴有低顺应性膀胱,5例表现高顺应性膀胱。术后3个月复查,72例均取得满意疗效。结论:BPH患者术前尿流动力学检测可以明确膀胱出口有无梗阻、逼尿肌顺应性和收缩功能,这对于合理选择术式提高疗效具有重要的临床意义。Objective： To evaluate the significance of urodynamic test in the preoperative diagnosis and treatment of BPH. Methods： 72 cases of BPH before urodynamics test, accorded to test results choosed a different method of treatment, which TURP 56 cases, 8 cases of line prostatectomy, 5 cases of cystostomy surgery, 3 cases of conservative treatment. Results： There were 72 cases of the existence of 57 cases of bladder outlet obstruction, 10 cases of suspected obstruction, 5 cases without obstruction. Detrusor contractility which weakened 13 cases, 35 cases of unstable detrusor function, 12 patients with low compliance bladder, 5 cases of high-performance compliance bladder. After 3 months, 72 cases are to achieve a satisfactory effect. Conclusion： BPH patients preoperative urodynamic test can clear whether bladder outlet obstruction, detrusor compliance and systolic function, which is a reasonable choice in order to enhance the effectiveness of surgical procedures have important clinical significance.
