摘  要:目的:探讨股骨头缺血性坏死的机理和治疗方法。方法:34例股骨头缺血性坏死Ⅰ、Ⅱ期患者,男28例,女6例;年龄34岁~53岁,平均45岁。患者术前均行X线和MR检查。病变累及髋关节左侧8例,右侧14例,双侧12例,共46个关节,按Ficat分期:Ⅰ期21例、Ⅱ期25例。采用股骨头髓芯减压术清除坏死骨,将同种异体骨与之前提取的骨髓基质干细胞附和,植入缺损区。采用股骨头缺血性坏死疗效评价法评估疗效。结果:34例患者术后无一例症状加重。平均手术时间120min,平均出血量200mL,术后随访时间12~18个月,平均14个月。16例Ⅰ期股骨头坏死恢复至正常,10例Ⅱ期恢复至正常,9例Ⅱ期恢复至Ⅰ期,其余均无进展。结论:骨髓基质干细胞附和同种异体骨移植是治疗股骨头缺血性坏死的一种安全有效方法。Objective:To discuss ANFH mechanism and treatment methods. Methods:34 patients of avascular necrosis of femoral head Ⅰ, Ⅱ Phase. There were 28 men and 6 women with an average age of 45 years (range 34 -53 years). Patients underwent pre - operative X - ray and MR examination. Lesion involving the left hip 8 cases, the right side of 14 cases, 12 cases of bilateral, a total of 46 joints. Ficat phases by: Ⅰ period of 21 cases, Ⅱ period 25 cases of. Core decompression of the femoral head adopted to remove necrotic bone, would be extracted prior to allogeneic bone and bone marrow stromal stem cells along, implantation defects. Avascular necrosis of femoral head was used as evaluation method to assess efficacy. Results:34 cases of patients with none of the symptoms increase. The mean operative time 120 minutes, average blood loss was 200mL, postoperative follow - up time 12 to 18 months, an average of 14 months. 16 cases of stage Ⅰ necrosis of the femoral head back to normal, 10 cases of stage Ⅱ back to normal, nine cases of stage Ⅱ returned to Phase Ⅰ, and the rest had no progress. Conclusion:Bone marrow stromal stem cells transplantation chime in allogeneic bone is the treatment of avascular necrosis of femoral head of a safe and effective method.
