摘  要:目的探讨青藤碱对糖尿病大鼠脑缺血再灌注损伤Bcl-2及Bax蛋白表达的影响。方法采用高脂高糖饮食加腹腔注射小剂量链脲霉素建立糖尿病大鼠模型,造模成功后将大鼠随机分为假手术组、缺血再灌注组、青藤碱低剂量治疗组和青藤碱高剂量治疗组。线栓法建立局灶性脑缺血再灌注模型。青藤碱低剂量(30mg/kg)、高剂量(60mg/kg)治疗组于术前30min分别给予大鼠腹腔注射。用免疫组化法观察缺血90min再灌注24h大鼠额顶部皮质Bcl-2和Bax蛋白表达,并进行2,3,5-三苯基氯化四氮唑(TTC)染色和HE染色观察脑梗死体积及病理形态学变化。结果(1)糖尿病大鼠脑缺血再灌注24h,缺血再灌注组Bcl-2和Bax表达增加,Bcl-2/Bax比值下降;与缺血再灌注组比较,青藤碱高、低剂量治疗组均显著增加Bcl-2表达,减少Bax表达,上调Bcl-2/Bax比值,高、低剂量组之间差异亦具有显著性。(2)青藤碱治疗组脑梗死体积较缺血再灌注组减小,高、低剂量组之间差异亦具有显著性。青藤碱治疗组脑组织缺血损伤病理学改变明显轻于缺血再灌注组,青藤碱高剂量组缺血改变亦轻于低剂量组。结论青藤碱对糖尿病大鼠缺血再灌注脑损伤具有保护作用,其机制可能与通过增加Bcl-2表达、减少Bax表达、上调Bcl-2/Bax比值有关。Objective: To study effect of sinomenine on expression of Bcl -2 and Bax of cerebral ischemia reperfusion(I/R)injury in diabetic rats. Methods:Rats' models of diabetes mellitus were made by high sucrose, fat diet and streptozotion injection. Diabetic rats were randomly divided into 4 groups, which were sham operated group, I/R group, low dose sinomenine group and high dose sinomenine group. The focal middle cerebral artery occlusion (MCAO) model was made by suture - occluded method. Sinomenine in low dose (60mg/kg) and high dose (60mg/kg) were given intraperitoneally to rats 30min before focal cerebral ischemia operation respectively . After 24h of reperfusion following ischemia for 90rain, the expressions of Bcl -2 and Bax were investigated with immunohistochemistry. HE staining and TFC staining was also investigated. Results: ( 1 ) The expressions of Bcl - 2 and Bax increased but the ratio of Bcl - 2 /Bax decreased in ischemia - reperfusion group. Compared with I/R group, low and high dose Sinomenine group increased expression of Bel - 2 and ratio of Bcl - 2 /Bax, but reduced expression of Bax. These differences were significance. (2) Compared with that of I/R group, cerebral infarction volume of Sinomenine treated group was decreased. It had significant difference between two groups. The change of ischemic impairment in Sinomenine treated group was lighter than that of I/R group, and high dose Sinomenine treated group was lighter than that of low dose Sin treated group. Conclusion: Sinomenine could reduced cerebral ischemia - reperfusion injure by increasing the expression of Bcl - 2 and decreasing that of Bax in diabetic rats. It plays protective effect from apoptosis through increasing the ratio of Bcl -2 /Bax in diabetic rats.
