摘  要:[目的]了解高水碘地区学龄儿童碘营养状况以及碘过量对儿童健康的影响。[方法]随机选择高碘地区2个乡4个村7~10岁125名儿童为调查对象,采集儿童晨尿和血,测定儿童尿碘、血清FT4、sTSH水平。采集当地饮用水及食用盐,测定水碘、盐碘含量。[结果]当地水碘在352.23~840.21μg/L范围内;调查两个地区食盐碘含量分别为(29.42±3.64)mg/kg和(35.46±4.01)mg/kg;受检儿童尿碘中位数为459.04μg/L,其中尿碘在200μg/L的占受检人数的94.4%,尿碘在800μg/L以上者占到20.8%;检出亚临床性甲状腺功能减退(亚甲减)儿童25例,占受检人数的20%;尿碘在200μg/L以下的儿童未检出亚甲减患者。[结论]该地区部分儿童碘营养水平适宜,大部分人群存在碘过量情况,碘摄入过多增加了亚临床甲状腺功能减退的危险性。故对高水碘地区人群的碘摄入应采取相应的措施以达到减少过量碘摄入的目的。[Objective] To investigate school children's iodine intake status in high water iodine areas and study the influence of excess iodine on schoolchildren's health. [Methods] 125 school children were chosen randomly from 4 hamlets of 2 towns which were high iodine areas in Shandong Province as donor. Blood and urine samples during morning fast were obtained. We used CLIA to measure sTSH, FT4, urinary iodine. We also measured the iodine amount in the drinking water and iodinated salt. [Results] The concentration of water iodine was 352.23-840.21μg / L, the salt iodine of the areas investigated was 29.42± 3.64 mg / kg, 35.46±4.01 mg / kg, respectively; the median of donors' urine iodine was 459.04μg / L, most urine iodine were higher than 200μg / L which accounting for 94.4%; and it included 25 donors who were diagnosed as subclinical hypothyroidism accounting for 20%, and the urine iodine of 20.8% were more than 800 μg / L. Donors whose urine iodine below 200μg / L ac-counted for only 5.6% without being diagnosed as subclinical hypothyroidism. [Conclusion] Some schoolchildren have a good iodine nutrition status and most of the schoolchildren in the areas investigated is excess, excess iodine intake elevate the risk of subclinical hypothyroidism, it needs some effective measure to limit the population's iodine intake in high iodine areas.
