乡镇查痰点发现涂阳肺结核病人方式的探讨

摘　　要：[目的]通过设置结核病乡镇查痰点,方便肺结核可疑症状者和疑似肺结核病人的就诊查痰,提高涂阳肺结核病人的发现率。[方法]乡镇卫生院对所辖区有肺结核可疑症状者和疑似肺结核病人免费进行痰涂片抗酸染色镜检初筛,凡痰涂片阳性病人或痰涂片阴性但高度怀疑肺结核者转至县级结防机构进一步检查确诊。县市(区)级结防机构对确诊肺结核病人经予免费抗结核治疗。建立激励机制:每例查痰的疑似肺结核病人经市、县两级结核病防治机构验收后给予工作人员补助9元。[结果]二年工作实施:登记疑似肺结核病人3973例,查痰2844例,检出涂阳病人130例,检出涂阳病人率4.57%。[结论]方便了边远的农村地区群众肺结核病人和肺结核可疑症状者就诊;是提高涂阳肺结核病人发现率的一条重要途径。[Objective] By setting up sites to check sputum for TB and provide convenience of checking sputum and treatment to suspicious persons with tuberculosis symptoms and suspected tuberculosis patients, to increase the rate of finding sputum smear-positive tuberculosis patients. [Methods] Screened suspicious person with tuberculosis symptoms and suspected tuberculosis patients through antacid stain examination of lens smeared with sputum at township hospitals, some sputum smear-positive patients and sputum smear-negative persons being suspected with tuberculosis were transferred to the county level TB prevention and treatment agencies for confirmation. The county level TB prevention and treatment agencies provided confirmed patients with the anti-tuberculosis treatment at free of charge. Established a mechanism of motivation： offered subsidy of nine Yuan to staff who took part in treating the cases of suspected tuberculosis patients found through checking sputum that were confirmed by the city and county level TB prevention and treatment agencies. [Results] The implementation of the program for two years： 3 973 cases of suspected tuberculosis patients were registered, 2 844 cases were checked for sputum, 130 cases were found to be the smear-positive patients, the rate of finding was 4.57 percent. [Conclusion] The program provides convenience for treatment of the tuberculosis patients and suspicious persons with tuberculosis symptoms in the remote rural areas; it is an important way to increase the rate of finding the smear-positive tuberculosis patients.
