特异性IgG4金标快速检测法在囊虫病诊断中的应用

摘　　要：目的创建简易、快速、敏感的囊虫IgG4检测方法，研制一种快速准确的囊虫病诊断试剂。方法以猪囊尾蚴囊液粗提液为检测用抗原，以胶体金标记抗人IgG4单克隆抗体为检测探针，采用自行设计的垂直流渗滤装置检测人血清中囊虫特异性IgG4抗体，平行检测IgG进行比较分析，并以囊虫IgG4 ELISA检测试剂为对照，评价其诊断价值。结果用金标渗滤法检测人血清中囊虫特异性IgG4的敏感性和特异性分别为95．7％（90／94）、100．0％（50／50），与IgG检测结果相近；与包虫病患者、裂头蚴病患者血清的交叉反应率分别为8．0％（4／25）、8．3％（4／24），低于IgG检测；未发现与肺吸虫病、血吸虫病患者血清有交叉反应。金标渗滤法快速检测囊虫IgG4在敏感性和特异性上略高于ELISA检测试剂，经70检验差异无统计学意义（X^2=2．77，P〉0．05）。结论金标渗滤法快速检测囊虫特异性IgG4，不仅操作简便、快速，而且敏感性高、特异性强，特别适合临床检测和现场查病，值得进一步开发应用。Objective To develop a simple, rapid and sensitive immunoassay and sero-immunological diagnostic kits for diagnosis of cystieereosis. Methods Crude extracts of cyst fluid of Cysticereus eellulosae were used as antigens and anti-human IgG4 McAb was conjugated with colloidal gold as the detecting probe. A special pad for vertieal-flow filtration assay was designed according to the reaction procedure to detect specific IgG4 or IgG against cytieerens in patient sera. ELISA kit for determination of eyticerens IgG4 was utilized as eontml to evaluate the new rapid assay. Results The sensitivity and specificity of this new rapid immunoassay for specific IgG4 against eystieerens were 95.7 % （90/94） and 100.0% （50/50）respectively, which were close to the results of IgG detection. There were 8.0% and 8.3 % of emss reactions with patient sera of echinoeoecosis and of sparganosis, lower than those of IgG detection. None cross reaetion showed with other helminthiasis such as sehistosomiasis and paragonimiasis infection. In eompared with Cyst-lgG4 ELISA kit the sensitivity and speeifieity of the DIGFA were higher slightly and there were no statistical differences according to chi-square criterion（ x^2 = 2.77, P 〉 0.05）, Conclusions This new immunoassay for detection of specific IgG4 is not only rapid and simple but also sensitivitive and speeificitive. It is very useful for elinieal diagnosis and field investigation of cysticercosis cellulosae.
