河北省农村酒精滥用和依赖性障碍的流行病学调查

摘　　要：目的了解河北省农村18岁及以上人群酒精滥用和依赖性障碍患病率及流行病学特点。方法采用多阶段、整群、分层随机抽样方法调查河北省农村18岁及以上人群共20696人,调查筛选工具采用改编后的一般健康问卷12项（GHQ-12）,以《DSM-IV-TR轴I障碍定式临床检查》（SC ID-T/P）病人版为调查的诊断工具,采用功能大体评定量表（GAF）评价功能状况。结果河北省农村酒精滥用和依赖性障碍时点患病率为20.16‰,终生患病率为36.78‰,女性为0.78‰,男性为39.60‰,男性明显高于女性（U=19.81,P〈0.01）。酒精滥用和酒精依赖性障碍以30-39岁、40-49岁、50-59岁患病率最高,分别为24.39‰、20.16‰、28.17‰,Log istic表明男性是最大的危险因素,是女性的48倍。结论酒精滥用和依赖性障碍发病率高、上升趋势明显,已经成为河北省农村精神疾病发病率最高的前4位疾病。Objective To understand the epidemilolgical characteristics of alcohol abuse and dependence obstacles in the 18- year-old and older men. Methods The multi-stage cluster and stratified random sampling method were Used to survey the 18- year-old and older men about 20696 in the countryside of Hebei. The tool of the revised edition of the General Heath Questionnaire （CHQ-12） for the investigation, Structural Clinical Interview for DSM-IV-7 TR Axis I Disorders-Patient Edition （SCID T/ P） for diagriosis and GAF for the analysis of the function status were adapted throughout the survey. Results The current prevalence of the alcohol abuse and dependence obstacles was 20. 16‰, lifetime incident was 36.78‰. The comparison also showed that current prevalence of 39.60‰ in male was higher than that of 0. 78‰ in female （u= 19.81, P〈0.01）. People ranged 30-39, 40-49, 50-59 years old showed higher disease incident than other ages, with relative rate 24.39‰, 20.16‰ and 28.17‰. Logistic analysis showed the risk factor of mate to the disease was 48 times of female. Conclusion The alcohol abuse and dependence obstacles with higher prevalence and obvious higher tendency has ranked the fourth of the most prevalent rate in Hebei.
