摘  要:目的分析新生儿窒息患儿早期血清酶活性变化与缺氧窒息程度的关系,探讨新生儿早期血清酶变化对评估窒息的临床意义和价值。方法回顾性分析本院2003年1月至2007年6月间89例新生儿窒息患者的生后3h内血清酶活性变化,比较轻度缺氧和重度缺氧新生儿血清酶水平的变化情况。结果生后3h内重度窒息组的血清酶(AST、LDH、CK、HBD)的均数均较轻度窒息组高,其中LDH存在明显差异,差异具有统计学意义(P<0.05),但AST、CK、HBD差异不甚明显,不具有统计学意义(P>0.05)。结论缺氧窒息可致血清酶活性增高,且增高程度与窒息程度有一定的关联性,但血清酶改变在生后数小时内还未达到高峰,故不能仅根据此时的酶学结果来评估脏器缺氧缺血性损害的程度,而需动态观察。Objective To explore the clinical significances and value of elevation in serum enzymes for asphyxia neonatorum in initial stage after parturition by analyzing the correlation of the changes in serum enzymes and the degree of asphyxia in newborn in the initial stage after birth. Methods Retrospective analyze the serum enzymes(AST,LDH,CK,HBDH) changes in initial 3 hours stage after birth for 89 asphyxia newborum cases , which were divided into two groups by Apgar score at 1 minute of age, severe asphyxia group (0 ≤ Apgar ≤ 3 ) and slight asphyxia group (4≤ Apgar ≤7 ). Compare the serum enzymes of both group. Result Compared with slight asphyxia group ,the level of serum enzymes incresed in severe asphyxia group. But only LDH showed significant difference. Conclusions Hypoxia and asphyxia can lead to serum enzymes increase. And the level of serum enzymes correlated with the degree of asphyxia. But the changes of the enzymes were not significant in the initial few hours. So we have to observe the serum enzymes dynamically.
