经腹及经阴道超声联合诊断未破裂型宫外孕的临床价值

摘　　要：目的:探讨经腹及经阴道彩色多普勒超声诊断早期异位妊娠的临床价值,旨在提高早期异位妊娠的诊断水平。方法:患者适当充盈膀胱,先经腹部检查,再经阴道检查,观察子宫附件及盆腔,寻找宫外孕包块,并进行有关测量。结果:48例经阴道彩超诊断早期异位妊娠,手术病理及保守治疗HCG随访证实46例,诊断符合率95.8%,其中5例为第2次异位妊娠,且2例为不孕症患者,3例合并宫内节育器。误诊2例,误诊率4.1%。结论:经阴道彩色多普勒超声结合腹部超声,早期诊断异位妊娠准确率高,有重要的临床意义。Objective： To evaluate the diagnostic value of transabdominal and transvaginal color Doppler ultrasonography in early ectopic pregnancy, for improving the diagnosis of early ectopic pregnancy. Methods： 48 cases underwent transabdominal and transvaginal color Doppler ultrasonography for observing uterine and its appendage and looking for lump of ectopic pregnancy. The related value was investigated. Results： 48 cases were diagnosed in ectopic pregnancy by transvaginal color Doppler ultrasonography, 46 of 48 cases were confirmed by pathological and the follow-up of HCG examinations . The coincidence rate of diagnosis was 95.8 % . 5 of 46 cases was ectopic pregnancy at second time. 3 cases had intrauterine contraceptive device. The rate of misdiagnosis was 4.1% . Conclusion： The transabdominal and transvaginal color Doppler ultrasonography is accurate and valuable in early diagnosis of ectopic pregnancy.
