摘　　要：目的:探讨血液和造血系统疾病的MRI表现。方法:选择经骨髓穿刺或活检证实的血液和造血系统疾病13例,其中急性白血病(acute leukemia,AL)7例,慢性淋巴细胞白血病(chronic lymphocyticleukemia,CLL)1例,非霍奇金淋巴瘤(non-Hodgkin lymphoma,NHL)2例,巨幼细胞性贫血2例,原发性血小板增多症(essential thrombocytosis,ET)1例。12例行胸腰椎骨髓MRI,1例行左胫腓骨MRI。4例先行MRI示信号异常,提示血液疾病,后经骨髓穿刺及活检确诊。MRI检查使用0.5T超导型磁共振成像系统,进行自旋回波(spin echo,SE)序列T1加权像(T1WI)、快速自旋回波(furbo spin echo,TSE)序列T2加权像(T2WI)、梯度回波(gradient echo,GRE)序列T2加权像(T2*WI),3例行短时反转恢复(short time inver-sion,STIR)序列。结果:13例患者受检骨髓MRI均表现为T1WI均匀低信号,T2WI 10例呈高信号,3例呈等信号,GRE序列T2*WI均表现为高信号,2例AL和1例NHL行STIR序列表现为高信号。1例AML-M2化疗达完全缓解后受检骨髓信号恢复正常,且髓外软组织浸润病灶消失。结论:MRI能为血液和造血系统疾病提供影像学依据,并有助于临床诊断和疗效的评价。Objective： To study MRI manifestation in of bone marrow hematological diseases. Methods： We choosed 13 cases of pathologically proved hematological diseases, including 7 cases of AL, 1 case of CLL, 2 cases of NHL, 2 cases of megaloblastic anemia and 1 case of ET. 12 cases of them had spinal marrow MRI, 1 case of them had left leg MRI. 4 cases were diagnosed as hematological diseases after MRI instection, which showed that their signals were not normal. MR imaging was performed with 0.5T superconductive MR scanner. T1 - weighted imaging with spin echo techniques, T2 - weighted imaging with furbo spin techniques, GRE and STIR were performed. Results： These 13 cases all showed homogenous low signal intensity on T1WI, and 10 cases of them showed high signal intensity on T2WI, 3 cases of them showed iso - signal intensity. All these 13 cases showed high signal on T2 ＊ WI of GRE. On STIR 2 cases of AL and 1 case of NHL showed high signal, lcases of AML- M2＇s spinal marrow＇s signal became normal when he completely recovered through treatment, and the soft tissue＇s infiltration disappeared too. Conclusion： The results suggested that knowledge of the MRI manifestation of hematological diseases were useful for the diagnosis and management of patients.
