摘  要:目的从分子水平上探讨红霉素(EM)的抗炎作用机制,为临床长期应用小剂量红霉素治疗慢性气道感染性疾病提供理论依据。方法体外培养人支气管上皮细胞(16 HBE),将细胞随机分为8组,先加入红霉素干预,后加入肿瘤坏死因子(TNF-α)刺激。分组如下:(1)空白对照组;(2)TNF-α(20ng/mL,16h);(3)EM(0.3μg/mL,24h)+TNF-α(20ng/mL,16h);(4)EM(3μg/mL,24h)+TNF-α(20ng/mL,16h);(5)EM(30μg/mL,24h)+TNF-α(20ng/mL,16h);(6)EM(0.3μg/mL,48h)+TNF-α(20ng/mL,16h);(7)EM(3μg/mL,48h)+TNF-α(20ng/mL,16h);(8)EM(30μg/mL,48h)+TNF-α(20ng/mL,16h)。然后收集各组细胞分别提取RNA,应用逆转录聚合酶链反应(RT-PCR)方法检测白介素-8(IL-8)mRNA。结果用前炎因子TNF-α刺激人支气管上皮细胞(16 HBE)后,RT-PCR结果显示细胞IL-8 mRNA表达增高。先加入不同浓度及作用时间的红霉素,后加入前炎因子TNF-α刺激人支气管上皮细胞(16 HBE),RT-PCR结果显示各组细胞IL-8 mRNA表达均降低,但其降低的水平与红霉素作用浓度及时间无明显关系。结论前炎因子TNF-α能激活人支气管上皮细胞IL-8基因的表达,使其表达增高,促进炎症的发生发展,在炎症进程中发挥着重要作用。红霉素能抑制人支气管上皮细胞白介素-8基因表达,这可能是红霉素的抗炎作用机制之一。Objective To investigate the anti-inflammatory molecular mechanism of erythromycin(EM) in order to provide a theoretical basis for long-term treatment of airway inflammatory diseasea with low-dose erythromycin. Methods Human bronchial epithelial cell line 16HBE was cultured with 10% serum DMEM(high glucose). The cells were randomly divided into eight groups such as (1)blank control group; (2)tumor necrosis factor (TNF-α) (20ng/mL, 16h) ; (3)EM(0.3μg/mL,24h) +TNF-α(20ng/mL, 16h) ; (4) EM(3μg/mL, 24h) +TNF-α(20ng/mL, 16h) ; (5)EM(30μg/mL,24h)+ TNF-α(20ng/mL, 16h) ; (6) EM(0.3μg/mL,48h) +TNF-α(20ng/mL, 16h) ; (7)EM(3μg/mL,48h) + TNF-α(20ng/mL, 16h) ; (8)EM(30μg/mL,48h) + TNF-α( 20ng/mL, 16h). The cells were incubated with various concentrations of EM for 24-48 hours before the stimulation by TNF-α. Then total cellular RNA was extracted to detect the expression of interleukin-8(IL-8) mRNA by applying reverse transcription polymerase chain reaction (RT-PCR). Results IL-8 mRNA was increased by the addition of TNF-α,which was inhibited by the preincubation with erythromycin. Conclusion Our results indicate that IL-8 mRNA was increased by the stimulation of TNF-α. Our results also indicate that erythromycin modify inflammation presumably by suppressing IL-8 mRNA in human bronchial epithelial cells.
