宫腔镜诊治妊娠终止后月经异常的临床体会

摘　　要：目的探讨宫腔镜检查在妊娠终止后出现月经异常的诊断和治疗价值。方法回顾性分析146例妊娠终止后出现月经异常患者的B超及宫腔镜检查结果,比较和分析B超与宫腔镜检查的诊断价值以及常规诊刮与宫腔镜治疗的疗效。结果宫腔镜检查和B超检查对月经异常原因的阳性发现率分别为91.7%和47.2%,两者对比差异有统计学意义(P<0.001);常规诊刮和宫腔镜对不全流产或子宫内膜息肉治疗的成功率分别为77.6%和96.2%,两者对比差异有统计学意义(P=0.032,P<0.05)。结论宫腔镜对妊娠终止后引起的细小的妊娠物残留、子宫内膜炎以及子宫内膜息肉、宫腔粘连和内膜过损,其确诊率明显优于B超,且可同时施予治疗,其疗效明显优于常规清宫。Objective To explore the diagnosis and treatment value of hysteroscope examination on irregular men-struation after termination of pregnancy. Methods 146 patients of irregular menstruation after termination of preguaney examined with B-utrasound or hysteroscope were analyzed and compared retrospectively. The routine curettage and the hystero-scope treatment effect of the patients were recorded and contrasted. Results The irregular menstruation pathogeny positive rate by hysteroseope and B-utrasound examination were 91.7% and 47.2% , respectively （P〈0.001）. Diagnosis success rate of incomplete termination or endometrial polyp by routine curettage and the hysteroscope were 77.6% and 96.2%, respectively （P〈0.05）. Conclusion The accuracy rate of hysteroscope is higher than that of B-ultrasound examination in the diagnosis of minute pregnancy leftover, endometritis, endometrial polyp,intrauterine adhesions and intima damage induced by abortion operation,all these diseases can be treated at the same time, and the effect of hysteroscope is better than that of routine complete curettage of uterine cavity.
