调Q-Nd:YAG激光及强脉冲光治疗面部雀斑的临床研究

摘　　要：目的比较调Q-Nd：YAG激光倍频532nm波长和强脉冲光560～1200nm波长治疗面部雀斑的疗效，随访观察两种方法治疗后的复发情况。方法将197例面部雀斑患者随机分为两组，调Q—Nd：YAG激光倍频532nm波长组84例和强脉冲光（IPL）560—1200nm波长组113例。两组均治疗1～3次，每次间隔时间4～8周。治疗结束后1—3个月判定临床疗效及不良反应。治疗结束后每6个月随访患者的复发时间及复发程度，两组共随访123例。结果调Q—Nd：YAG激光倍频532nm波长组治愈67例（79．76％），平均治愈次数1．33次，复发16例（33．33％），平均复发时间2．25年；IPL560～1200nm波长组治愈40例（35．40％），平均治愈次数2．15次，复发44例（58．67％），平均复发时间1．36年。两组治愈率和复发率比较，差异有显著性（P均〈0．001）。结论Q—Nd：YAG激光532nm治疗面部雀班较IPL560—1200nm治愈率高，复发率低，复发时间长。Objective To compare the curative efficacy of facial freckles treated by Q-swiched Nd：YAG laser （frequency double） with wave length 532nm and intense pulse light with wave length 560 - 1 20Ohm , then followed up the relapse of each group. Methods 197 patients with freckles were divided into two groups randomly. 84 cases were treated by Q-swiched Nd：YAG laser（frequency double） with wave length 532nm and 113 cases by intense pulse light with wave length 560 - 1 200nm. Each group was treated 3 times and interval was 4 - 8 weeks. Curative effect was observed after three months. 123 cases were traced more than 6 months. Results In Q-swiched Nd：YAG laser（frequency double） with wave length 532nm group 67 （79.76%） cases were cure completely, average curing times was 1.3 days, 16 （33.33 % ） cases recurred, median relapse time was 2.25 years; IPL560 - 1 200nm group 40（35.40% ） cases were cure completely, average curing times was 1.7 days, 44（33.33% ） cases re- curred, median relapse time was 1.36 years. There was a significant difference between rate of recovery and relapse in two groups, （P 〈0. 001 ）. Conclusion Freckles treated by Q-swiched Nd：YAG laser （frequency double） with wave length 532nm have higher healing rate and lower recurrent rate and longer recurrence time than those treated by IPL 560 - 1 200nm.
