摘 要:目的评价CO2气腹及不同气腹压力对子宫内膜异位症病灶基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶抑制剂-1(TIMP-1)蛋白表达的影响。方法建立SD大鼠子宫内膜异位症模型,造模成功后随机分组:对照组、10mmHg和20mmHgCO2气腹组,气腹作用时间1h。气腹干预后1周、2周和6周检测子宫内膜异位病灶MMP-2、TIMP-1蛋白表达情况。结果与对照组比较,气腹干预后1周10mmHg和20mmHgCO2气腹组子宫内膜异位病灶MMP-2蛋白表达均显著降低(P<0.001),且10mmHg组低于20mmHg组(P<0.001),可持续至2周以后。两气腹组TIMP-1表达显著增强,且10mmHg组高于20mmHg组(P<0.001)。MMP-2/TIMP-1比例下降,异位病灶组织侵袭能力下降。随着时间变化,两组MMP-2逐渐升高,TIMP-1下降,术后6周与对照组比较,差异无统计学意义(P>0.05)。结论CO2气腹作用后早期可使子宫内膜异位灶组织MMP-2下降,TIMP-1升高,侵袭力下降,低气腹压力对子宫内膜异位病灶侵袭力的抑制作用更为明显。Objective To evaluate the effect of CO2 different pneumoperitoneum pressure on protein expression matrix metalloproteinase - 2 (MMP - 2) and tissue inhibitor of metalloproteinase - 1 (TIMP - 1) of endometriosis lesion. Methods The SD rats were divided into randomly three groups: control group, 10 mmHg and 20 mmHg CO2 pneumoperitoneum after endometriosis model being founded successfully. The pressure continued for about lb. The MMP - 2 and TIMP -1 in EMs lesion were detemined at 1w, 2w and 6w. Results Compared with the control group, the protein expression level of MMP - 2 in the two CO2 pneumoperitoneum groups were decreased significantly in post-operation one week (P(0. 001). And the effect could be continued until two weeks after pneumoperitoneum intervention. Moreover the expression level of MMP- 2 in 10mmHg group was lower than that in 20 mmHg group pneumoperitoneum (P〈0. 001). The protein expression of TIMP- 1 increased in CO2 groups, with the higher expression in 10mmHg group (P〈0. 001). The expression level of MMP- 2 was increased and TIMP- 1 was decreased gradually with time after CO2 pneumoperitoneum intervention, and there were no difference compared with that of the control group six weeks post - operation (P〉0.05). Conclusion The expression of MMP - 2 is decreased and TIMP- 1 is increased in the early period CO2 pneumoperitoneum intervention. The low CCh pneumoperitoneum pressure is superior to the high pressure group in inhibition of aggression of EMs lesion.
