参松养心胶囊联合卡维地洛对慢性心力衰竭患者血浆脑钠肽的影响

摘　　要：目的观察参松养心胶囊联合卡维地洛治疗慢性心力衰竭(CHF)的临床疗效、安全性及对血浆脑钠肽(BNP)的影响。方法108例慢性心力衰竭患者随机分为两组。对照组予利尿剂、血管紧张素转换酶抑制剂、正性肌力药物、扩血管药物及营养心肌等药物治疗。治疗组在此基础上加用参松养心胶囊和卡维地洛口服。两组治疗8周后观察临床症状、心率、血压、心功能分级、左室舒张末期内径(LVEDd)、左室射血分数(LVEF)及BNP的变化情况。结果治疗组治疗后心功能明显改善,总有效率为91.07%,对照组总有效率69.23%(P<0.01),治疗后LVEF明显提高(P<0.01),LVEDd较对照组有更明显的降低(P<0.01),其血浆BNP水平下降较对照组更为明显(P<0.05)。结论参松养心胶囊联合卡维地洛治疗CHF安全有效且副反应小。Objective To observe the effect of shensongyangxin capsule combined with carvedilol on plasma levels of brain natriuretic peptides （BNP） and cardiac function in patients （pts） with chronic heart failure （CHF）. Methods One hundred and eight pts with chro nic heart failure were divided randomly into two groups： Control group treated with diuretic, ACE inhibitor and digitoxin,and treatment group treated with Shensongyangxin capsule and carvedilol on the basis of above regular treatment. All pts were followed up for 8 weeks. The clinical symptoms, heart rate, blood pressure, heart function,left ventricular ejection fraction（LVEF）, left ventricular end diastolic diameter（LVEDd） and plasma levels of BNP were studied. Results After treatment, total effective rate in the treatment group and the control group was 91.07 % and 69.23 % respectively（P〈0.01）. All index in treatment group was obviously improved. LVEF in treat ment group was higher than that in control group（P〈0.01） ,while LVEDd in treatment group was decreased than that in control group（P〈0.01）. Level of plasma BNP was decreased significantly（P〈0.05）. Conclusion Shensongyangxin capsule combined with carvedilol was effective for treatment of chronic heart failure.
