摘 要:目的探讨原发性肾小球肾炎(PGN)患者血、尿β2-微球蛋白(β2-MG)浓度与肾脏病理变化的关系。方法对象为300例PGN患者。采用放射免疫法测定血、尿β2-MG浓度。将上述指标与肾脏病理评分进行统计学分析。结果与其他病理类型的PGN患者比较,硬化性肾小球肾炎(SGN)患者血、尿β2-MG浓度最高(P均〈0.01)。血β2-MG分别与肾小球硬化程度、间质纤维化、肾小管萎缩、间质炎症细胞浸润呈正相关(P均〈0.01)。尿β2-MG与间质纤维化、间质炎症细胞浸润呈正相关(P均〈0.01),也与肾小球硬化程度呈正相关(P〈0.05),与肾小球系膜增生程度呈负相关(P〈0.05)。肾小球硬化程度、间质纤维化、肾小管萎缩是血β2-MG的独立危险因素。间质纤维化、肾小球系膜增生程度是尿β2-MG的独立危险因素。结论不同的病理类型血、尿β2-MG的浓度是不同的。肾小管间质损害和肾小球硬化程度是影响血、尿β2-MG的主要因素。尿β2-MG浓度也与肾小球系膜增生程度的关系密切。Objective To investigate the correlation between beta-2 microglobulin (β2-MG)and renal pathology in primary glomerulonephritis (PGN) patients. Methods Three hundred PGN patients without any special treatment were enrolled in this study. All these patients underwent renal biopsy. Serum and urinary β2- MG were measured before the biopsy. Results Compared to patients with other pathological types, Serum and urinary β2-MG were the highest in patients with selerosing glomerulonephritis ( SGN ) ( respectively, P 〈 0.01 ). Serum β2-MG was positively correlated with degree of glomeruloselerosis, degree of tubular atrophy, degree of interstitial fibrosis and degree of interstitial infiltration ( respectively, P 〈 0.01 ). Urinary β2-MG were positively correlated with degree of interstitial fibrosis and degree of interstitial infiltration ( respectively, P 〈 0. 01 ), also correlated with degree of glomerulosclerosis( P 〈 0. 05 ) , and negatively correlated with degree of mesangial matrix expansion( P 〈 0. 05 ). Degree of glomeruloselerosis, degree of tubular atrophy and degree of interstitial fibrosis were the independent risk factors of the serum β2-MG. Degree of interstitial fibrosis and degree of mesangial matrix expansion were the independent risk factors of the urinary β2-MG. Conclusions There are differences of serum and urinary β2-MG respectively among PGN patients with different pathological types. Renal tubulointerstitial damage and degree of glomeruloselerosis are an importamt influencing factors of serum and urinary β2-MG. Urinary β2-MG is also a significantly correlated with degree of mesangial matrix expansion.
