纵轴超声内镜检查在上消化道隆起性病变中的诊断与鉴别诊断价值

摘　　要：目的:评估纵轴超声内镜在上消化道隆起性病变中的诊断与鉴别诊断价值。方法:用纵轴超声内镜对145例上消化道隆起性病变进行检查,19例行超声内镜引导下细针穿刺吸取术(EUS-FNA)。结果:经EUS诊断上消化道隆起性病变中,胃肠间质瘤54例,平滑肌瘤38例,平滑肌肉瘤1例,息肉6例,异位胰腺7例,脂肪瘤4例,静脉瘤4例,囊肿2例,壁外压迫29例。经病理证实61例。结论:超声内镜能清楚显示消化道管壁的层次,对上消化道隆起性病变的诊断与鉴别诊断具有较高价值。Objective：To examine the clinical significance of endoscopic uhrasonography（EUS） in the detection and differential diagnosis of protu- berances in upper gastrointestinal tracts. Methods ： One hundred and forty-five patients with estimated protuberances in upper gastrointestinal tracts underwent teh screening examination by EUS, in which 19 received additional endoscopic uhrasonagraphy guided fine needle aspiration（ EUSFNA ）. Results ： By EUS, the protuberances were confirmed to be gastrointestinal stromal tumor in 54 cases, leiomyoma in 38, leiomyosarcoma in 1, polypus in 6, aberrant pancreases in stomach in 7, gastric lipoma in 4, phlebangioma in gastric fundus and esophagus in 4, esophageal cysts in 2 and result of extraluminal compression in 29. In 145 Cases 61 were identified by histopathological observation. Conclusion ： Longitudinal EUS is of high value in differential diagnosis of protuberances in the upper gastrointestinal tracts, for it can exactly distinguish the mucosal layers.
