摘 要:目的:探讨急性期QT间期离散度(QTd)在川崎病冠状动脉损伤中的临床意义。方法:对40例川崎病(KD)患儿在发病急性期进行心电图检查,同时做心脏超声心动图观察冠状动脉损伤情况,并在1个月内复查心脏超声心动图,将有冠状动脉损伤的作为损伤组,无冠状动脉损伤的为非损伤组,同时随机抽取在此期间有发热的非KD患儿34例为对照组,比较各组间QTd值及与冠状动脉损伤的相关性。结果:冠状动脉损伤组和非损伤组川崎病患儿急性期QTd值(53.23±15.07,31.41±16.46)均显著延长,两组有统计学差异(P<0.05);冠状动脉损伤组、非损伤组与对照组(22.71±9.47)比较差别均有显著性意义(P<0.05)。川崎病冠状动脉损伤同急性期QTd值呈正相关(rs=0.551,P(0.05)。结论:观察川崎病患儿急性期QTd值,对川崎病冠状动脉损害的早期诊断有重要意义。Objective: To explore the clinical significance of QT dispersion(QTd) in diagnosis of Kawasaki disease(KD) complicated with coronary artery lesions at acute stage of onset. Methods : Forty children with KD at the acute stage onset underwent electrocardiogram(ECG) and echocardiography for examining the degrees of lesions, with rechecking within 1 month after the diagnosis. Those with or without coronary artery lesions were considered as lesion or non-lesion group, respectively. In addition, 34 cases of non-KD but with fever were chosen as controls for comparing the QTd findings and degrees of coronary artery lesions. Results: The value of QTd(53.23 ± 15.07 ) in coronary artery lesion group were higher than that(31.41 ± 16.46) of without coronary artery lesions( P 〈 0. 05 ). The same findings were observed in both lesion and non - lesion groups as compared with the controls (22 . 71 ± 9.47 ) ( P 〈 0.05 ). The value of QTd in cases of KD with coronary artery lesions were positively correlated with the acute stage onset ( rs = 0. 551, P 〈 0. 05 ) Conclusion : The QTd value may reveal the degree of coronary artery lesions in children with KD during the acute stage onset and plays an significant role in early diagnosis of this disease.
