硝普钠与卡托普利联合应用治疗动脉导管未闭术后高血压

摘　　要：目的：探讨卡托普利与硝普钠联用对动脉导管未闭（ＰＤＡ）病儿手术后并发高血压的疗效。方法：ＰＤＡ手术后血压升高者１３２例，分２组。Ａ组８５例（男性３４例，女性５１例）为术后血压升高≤４ｋＰａ者；Ｂ组４７例（男性１８例，女性２９例）为术后血压升高＞４ｋＰａ者。术后均即用硝普钠１～５μｇ／（ｋｇ·ｍｉｎ）连续静脉微泵推注１２～４８ｈ；术后１２ｈ开始用卡托普利０．５～１ｍｇ／ｋｇ，ｐｏ，ｂｉｄ用２～４ｗｋ。结果：Ａ组用硝普钠为１８±ｓ３ｈ，卡托普利为１０±４ｄ，Ｂ组依次为３２±４ｈ与２２±４ｄ，才获降压至正常，组间比较，Ｐ＜０．０５；有效率Ａ组为１００％，Ｂ组为９８％，组间比较，Ｐ＞０．０５。结论：２药合用效果良好。术后血压升高＞４ｋＰａ者起效慢。AIM: To study the effect of sodium nitroprusside combined captopril in postoperative hypertension in patients with patent ductus arteriosus (PDA). METHODS: Patients after operation on PDA showing hypertensive reaction, systolic blood pressure (SBP) was increased 10% as compared with preoperation in 132 patients, they were divided into 2 groups. Group A: Increase of SBP≤4 kPa in 85 patients (M 34, F 51; age 5.6±s 0.9 a); Group B: Increase of SBP>4 kPa in 47 patients (M 18, F 29; age 5.3±0.8 a). They were given sodium nitroprusside 1_5 μg/(kg·min) by iv perfusion with an electronic pamp, and 12 h after the operation, they were given captopril 0.5_1 mg/kg, po[WTBZ], bid, for 2_4 wk. RESULTS: In group A and group B, the time for sodium nitroprusside to control the SBP were 18±3 h and 32±4 h; the time for captopril to control the SBP were 10±4 d and 22±4 d(P<0.05); and the effective rates were 100% and 98% (P>0.05) respectively. CONCLUSION: Sodium nitroprusside followed by captopril in treating postoperative hypertension in patients with PDA is good, but the group B is slow in take effect.
