摘  要:目的探讨细胞学检查对不明原因肺部疾病诊断的价值。方法对65例门诊及住院患者,经X线和(或)CT检查提示肺部有异常阴影或肺不张,而经临床相关检查不能明确诊断的患者行支气管镜检查,获取病理和细胞学标本并对细胞学检查结果进行分析。结果65例中细胞学诊断为恶性肿瘤者28例,占43.08%,其中鳞癌14例、腺癌4例、小细胞癌3例、不能分类7例;炎症31例,占47.69%;其他诊断6例,占9.23%。病理诊断为恶性肿瘤者31例,占47.69%,其中鳞癌者18例、腺癌6例、小细胞癌1例、不能分类6例;炎症25例,占38.46%;结核1例,占0.15%;其他诊断者8例,占12.31%。结论对肺部病变通过其他常规检查或痰细胞学检查不能确定诊断者以支气管镜检查获取病理、细胞学标本是帮助诊断不可缺少的手段之一。Objective To discuss and analyze the value of cytologic examination for unknown cause pneumonosis. Methods To summarize the results of cytology and pathology for 65 out - patients and in - patients patients with abnormal shadow or pulmonary atelectasis in lungs by X - rays or CT. They could not be finally diagnosed by relatively clinic examinations, but they were performed bronchoscopia to obtain the pathologic specimens. Results Among 65 patients, there are 28 cases (43.08%) of malignancy, that includes 14 cases of squamous carcinoma, 4 cases of adencarcinoma, 3 cases of anaplastic carcinoma and 7 cases of indeterminate type tumor, 31 cases (47.69%) of inflammation and 6 cases (9.23%) of other diseases by cytologic diagnosis. Among 65 patients, there are 31 cases (47.69% ) of malignancy, that includes 18 cases of squamous carcinoma, 6 cases of adencarcinoma, 3 cases of anaplastic carcinoma, 6 cases of indeterminate type turnout, 25 cases (38.46 % ) of inflammation, f cases (0.15 % ) of tuberculosis and 8 cases ( 12.31% ) of other diseases by pathological diagnosis. Conclusion Cytological and pathological examinations, whose specimens are obtained by branchoscopia, are sine qua non diagnostic methods for unknown cause pneumonosis which can't be finally diagnosed by routine methods or sputum cytology examination.
