应用腔镜甲状腺手术拉钩70例分析

摘　　要：目的观察腔镜甲状腺手术拉钩在腔镜甲状腺手术中的使用技巧及注意事项。方法总结应用腔镜甲状腺手术拉钩施行的腔镜甲状腺手术70例的临床资料。结果本组在腔镜甲状腺手术拉钩的帮助下完成70例腔镜甲状腺手术,无中转开放手术。手术时间80～130分钟,平均100分钟,出血量约50ml。术后无声嘶、呛咳、出血、手足抽搐、皮下积气、皮下积液及窒息等并发症,腔镜甲状腺手术拉钩进入孔处术后无疤痕,效果满意。结论应用腔镜甲状腺手术拉钩行腔镜下甲状腺手术具有操作简单、手术空间增大、省时、出血少、损伤小、拉钩进出孔处皮肤无疤痕存留等特点,有临床推广应用价值。Objective To evaluate the technique and points of the specialized endoscopic thyroidectomy＇s retractor when it is used in laparoscopic thyroidectomy. Methods 70 cases of laparoscopic thyroidectomy with the specialized retractor were analysed and summarized. Results All of 70 cases were successfully finished on laparoscopic thyroidectomy with the specialized retractor, 0 cases were reversed to open surgery. The operation time was 80~130 minutes,and average time was 100 minutes,with hemorrhage of 50 mL during the surgery. After surgery no complications such as massive husky voice ,hemorrhage, twitch of hand and foot, pneumohypoderma and asphyxia. After the operation no scar emerges where the specialized endoscopic thyroidectomy＇s retractor pass through the skin. Conclusion Application of the specialized rectractor can make the operation easily, expand the visual field,save time, decrease haemorrhage, minimize the invasion.The specialized rectractor deserves clinical application and dissemination.
