摘 要:目的:比较冠状动脉结扎术与腹主动脉缩窄术所建立的大鼠慢性心力衰竭(CHF)模型在病理及病理生理学等方面的差别。方法:实验大鼠随机分为4组:正常对照组(假冠状动脉结扎术组S-LCL、假腹主动脉缩窄术组S-AAC)、冠状动脉结扎术组(LCL组)和腹主动脉缩窄术组(AAC组),8周后测定血流动力学变化,计算左心室重量/体重比值(LVW/BW)观察心肌组织形态学改变。结果:LVW/BW在LCL组、AAC组均增加。两种CHF模型左室压力上升和下降最大速率(±dp/dt m ax)均较正常对照组降低(P<0.01)。左室收缩压(LV SP)、左室舒张末压(LVEDP)均较正常对照组升高,而且AAC组比LCL组升高更明显。光镜下LCL组梗死区纤维组织替代坏死心肌细胞,大量炎性细胞浸润,非梗死区心肌细胞肥大伴间质显著增生;AAC组心肌细胞肥大,排列紊乱,小血管壁增厚,间质增生伴少量炎细胞浸润。结论:冠脉结扎术和腹主动脉缩窄术均是建立慢性心力衰竭大鼠模型的有效方法。两者的差别是:腹主动脉缩窄术建立的是压力超负荷型CHF模型,冠脉结扎术建立的是低心输出量型CHF模型。Objective:To compare the pathologic and pathophysiologic characteristics of two experimental models of chronic heart failure (CHF) established by coronary artery ligature and abdominal aorta constriction separately. Methods : Wistar rats were randomly divided into four groups : AAC,S-AAC,LCL and S-LCL groups. Coronary artery ligature and abdominal aorta constriction were induced respectively in group LCL and AAC to establish models of CHF. The changes in hemodynamic parameters, left ventricular weight/body weight (LVW/BW) ratio, cardiomorphology were investigated after eight weeks. Results : Both coronary artery ligature and abdominal aorta constriction established experimental models of CHF successfully. LVW/BW were increased in group AAC and LCL. Compared with S-AAC,S-LCL groups, ± dp/dt max in both groups AAC and LCL decreased significantly (P〈 0.01). LVSP,LVEDP in both groups AAC and LCL increased significantly (P〈0.05, P〈0. 01). The increase of LVSP,LVEDP in group AAC was higher than that in group LCL (P〈0. 01). Pathology examination in group LCL showed that in infarcted area cardiomyocytes were replaced by fibrous tissues with inflammatory ceils infiltration after coagulation necrosis, while in noninfarcted area hypertrophic cardiomyocytes were accompanied by hyperplasia of stroma. In group AAC, hypertrophic cardiomyoeytes, thickening of small vessel wall and hyperplasia of stroma with inflammatory cells infiltration were seen. Conclusion :Both coronary artery ligature and abdominal aorta constriction are useful techniques to establish experimental models of CHF. The models made by abdominal aorta constriction are pressure-overloading CHF model, while the models made by coronary artery ligature are low cardiac out put CHF model.
