摘 要:目的利用64层CT各向同性特点探讨成人骨性鼻泪管多层螺旋CT解剖结构。方法对43例(86侧)成人骨性鼻泪管区容积扫描,在其薄层MPR重组图像上测量骨性鼻泪管部分径线及角度,并观察管腔内及内外骨壁情况。结果成人骨性鼻泪管上端左右径(5.2±0.8)mm,上端前后径(5.8±0.8)mm,中部前后径(4.5±0.9)mm,前后径(5.9±1.2)mm,下端左右径(5.0±0.8)mm,前后径(7.6±1.1)mm,上下径(12.6±2.3)mm,孔口距(30.2±2.4)mm,与水平面及矢状面间夹角分别为(60.4±6.7)°、(3.7±2.6)°。所有测量数据左右间差异均无统计学意义(P0.05),男女间在骨性鼻泪管上口前后径、上下径及孔口距差异有统计学意义;鼻泪管且自上而下有向外侧倾斜(63%),向内斜(24%)和平行走行(13%);大部分骨性鼻泪管内侧骨壁为薄壁(72%);有11侧鼻泪管内含气(13%)。结论64层螺旋CT薄层MPR图像是显示骨性鼻泪管解剖结构和空间走行简易可靠影像学方法。Objective Utilizing the isotropic 64 slices spiral CT scanning to know the anatomy ofnasolacrimal duct. Method Forty three cases of healthy adults were underwent CT volume scan in nasolacrimal ducts region, the involved distances and angles of the nasolacrimal ducts were measured on MPR images, and the bone walls were observed and analyzed. Results The left-right and anteroposterior diameter of the superior orifices was (5.2±0.8)mm and (5.8±0.8)mm respectively. The left-right and anteroposterior diameter in the middle part of the nasolacrimal ducts was (4.5±0.9) mm and (5.9±1.2)mm respectively. The left-right and anteroposterior diameter of the inferior orifices was (5.0±0.8)mm and(7.6±1.1)mm respectively. The The nasolacrimal duct length was (12.6±2.3)mm. The distance from the inferior margin of the anterior nostril to the anterior margin of the orifice (DNO) was (30.2±2.4)mm. The angle between the nasolacrimal duct and horizontal or vertical plane was (60.4±6.7)°and (3.7±2.6)° respectively. There was no significant difference between left and right side. There were significant differences between male and female in anteroposterior diameter of the superior orifice, vertical diameter, and the DNO. The nasolacrimal ducts can tilt outward(63%), inward(24%), or parallel to the middle sagittal plane(13%). Most nasolacrimal ducts have thin medial bone wa11(72%). There was gas in the nasolacrimal ducts for 11 cases(13%). Conclusion The thin slice MPR images of 64 slices spiral CT may be used as a simple and reliable method to observe the nasolacrimal ducts.
