头皮毛囊癌MRI诊断与治疗观察(附3例报告)

摘　　要：目的:探讨毛囊癌的MR诊断及治疗后情况。方法:回顾性分析3例毛囊癌患者的MR表现、病理资料及手术治疗情况,3例均为女性,年龄49～79岁。MR表现主要为头皮类圆形囊实性混杂信号影,肿块有包膜,肿块周围骨质侵蚀,囊性肿块旁实性部分有不同程度骨质侵犯,肿块周围水肿明显。注入Gd-DTPA后肿块实性部分及包膜强化明显,肿块边缘脑膜强化明显。3例患者均手术治疗和病理证实。结果:1例患者术后6月复发,再次手术后3个月复发,最终死于肿瘤转移;1例术后随访1年复发,再次手术6个月后原位复发,术后随访至今未见复发和转移;1例随访3年均为复发和转移。结论:毛囊癌发病率极低,确诊需靠组织病理学及免疫组织化学检查。但MRI对发生周围侵犯及淋巴结转移有一定的诊断价值。治疗以手术切除为主,手术中注意切口及周围组织的保护。Objective：To study MR diagnosis and condition after the trcatment of trichocarcinoma. Methods：MR appearances, pathological information and the condition after the treatment were analyzed retrospectively in three female patients with trichocarcinoma, who were 49 to 79 years old. Mixed circular signal of scalp, tumor＇s amiculo, ossea erode, peri-tumural edema were major MR appearance. The substance, amiculo and nearly meninx of tumor were strengthened obviously after injecting of Gd-DTPA. Three patients were all proved by surgery arid pathology. Results： First patient recurred in 6 months after the operation and it relapsed again after another surgery three month later. At last, he died of metastasis. The recurrence happened in a year after the surgery in second patient, who then recurred again six month later after another operation. After one more operation, follow-up showed that no recurrence and metastasis happened again. Follow-up showed that the tumor had not any recurrence and metastasis in third patient. Concluslon：The incidence rate of Trichocarcinoma is very low. Histopathology and immuo-histochemistry examinations can diagnose the disease. MRI has a certain diagnostic value on surrounding encroachment and metastasis of lymph node. The operation is the main means of the treatment. Surgeons should pay more attention to the cut edge and preserve the normal tissue during the operation.
