高危老年病人髋关节手术的麻醉处理及围手术期治疗

摘　　要：目的：本文分析54例70岁以上合并心血管疾病高危老年病人髋关节手术的硬膜外麻醉及围术期治疗。方法：54例合并心血管疾病的高危老年病人，围术期对心功能的评估，支持和相应的处理，改善心功能．采用硬膜外麻醉，用0．5—0.75％罗哌卡因麻醉，麻醉平面控制在T10水平，观察硬膜外给药后5、10、20、30分钟BP、HR、SpO2的变化，麻醉效果和麻醉并发。结果：全部病人镇痛良好，肌肉松驰，效果满意，对呼吸循环影响小，围术期无死亡。结论：硬膜外麻醉用于合并心血管疾病高危老年病人髋关节手术。加强麻醉监测和管理与围术期要特别注意对心功能的支持，维护及合理用药是重要保证。objective： To anatyze the effect of epidural anesthesia in the hip joint surgery of high-risk old people. Methods： Treating 54 cases of high-risk old people with epidural anesthesia and observing the changes of BP. HR. SpO2 5, 10, 20, 30 minutes after the anesthesia respectively as well as the effects of anesthesia. Results： The effect was satisfactory and no one died during the perioperative. Conclusion： Epidural anesthesia is effective in the hip joint surgery of high-risk old people. We should strengthen the anesthesia examination and management as well as the support of perioperative treatment.
