艾司洛尔与硝酸甘油合用对吸痰患者心血管反应的预防

摘　　要：目的探讨艾司洛尔复合硝酸甘油滴鼻对吸痰患者心血管副反应的预防作用。方法80例气管切开患者随机均等分为对照组、艾司洛尔组、硝酸甘油组及艾司洛尔复合硝酸甘油组。记录给药前、给药后、吸痰即刻及吸痰后1、5min各时段SBP、DBP、HR的变化。结果在吸痰期,艾司洛尔组患者心率变化与对照组比较明显降低(P<0.01),血压变化与对照组比较略降低,但无差异;硝酸甘油组患者血压较平稳,与对照比较心率变化无显著性差异(P<0.01);艾司洛尔复合硝酸甘油组血压、心率均明显较对照组平稳(P<0.01)。结论吸痰前1min静注1.5mg/kg艾司洛尔,加吸痰前3min滴鼻0.2%硝酸甘油4μg/kg可以抑制吸痰时发生的心血管副反应。Objective To explore the prevention role of composites esmolol nitroglycerin in patients with nasal suction on the cardiovascular side effects. Methods 80 cases of tracheotomy patients were randomly divided into two equal in the control group,esmolol group,the nitro-glycerin group and esmolol composite nitroglycerin group. The change of the SBP, DBP and HR of the administration before delivery,suction and immediately after the suction 1,5min were recorded. Results In the suction period,esmolol patients heart rate decreased significantly compared with the control group（P〈0.01）, compared with the control of blood pressure changes slightly lower,but no differences in blood pressure in patients with nitroglycerin group more stable, compared with the control（P 〈0.01）, no difference in heart rate;Esmolol composite group of nitroglycerin blood pressure and heart rate were stable than the control group （P〈 0.01）. Conclusion lmin before the suction 1.5mg/kg intravenous esmolol, plus pre-suction 3min nose 0.2% nitroglycerin 4μg/kg when the suction can inhibit cardiovascular side effects.
