摘  要:目的研究器官移植受者T淋巴细胞亚群分类与人巨细胞病毒活动性感染的关系。方法使用免疫荧光技术对2006年9月-2008年1月采集的27例器官移植受者外周血的样本pp65抗原进行检测,同时使用流式细胞技术上述样本的T淋巴细胞亚群进行分析。结果非活动性感染组与活动性感染组CD3+淋巴细胞含量分别为(75.96±8.18)%和(67.75±17.39)%,差异无统计学意义(P>0.05);非活动性感染组与活动性感染组CD4+淋巴细胞含量分别为(53.37±4.38)%和(29.25±9.38)%,非活动性感染组高于活动性感染组,差异有统计学意义(P<0.01);非活动性感染组与活动性感染组CD8+淋巴细胞含量分别为(20.73±4.45)%和(36.12±15.60)%,非活动性感染组低于活动性感染组,差异有统计学意义(P<0.01);非活动性感染组与活动性感染CD4+/CD8+比值分别为2.64±0.53和1.07±0.45,非活动性感染组高于活动性感染组,差异有统计学意义(P<0.01)。结论非活动性感染组与活动性感染组中CD3+淋巴细胞百分含量差异无统计学意义,即T淋巴细胞的含量差异无统计学意义,因此单纯检测CD3+淋巴细胞对鉴别诊断无使用价值;HCMV抗原在体外主要刺激CD8+淋巴细胞增殖,活动性感染组中外周血CD8+淋巴细胞高于非活动性感染组,致使CD4+淋巴细胞与CD8+淋巴细胞比值下降,甚至倒置。OBJECTIVE To investigate the relation between T lymphocyte subpopulation and the HCMV active infection. METHODS The HCMV pp65 antigen from 27 peripheral blood samples was collected from 27 organ transplant recipients between Sep 2006 and Jan 2008 and tested via immunofluorescence assay. T lymphocyte subpopulation of the samples was detected by flow cytometry. RESULTS The Cd3^+ lymphocyte in the HCMV unactive infection group and the HCMV active infection group was (75. 96 ±8. 18 ) % and ( 67. 75 ± 17.39 )%, respectively, and there was no sigmificant difference between two groups (P〉0. 05). The CD4^+ lymphocyte of them was (53.37±4.38)% and(29. 25 ±9. 38) %, respectively, and the difference between two groups was significant (P〈 0. 01). The CD8 + lymphocyte in HCMV unactive infection group and the HCMV active infection group was (20. 73± 4.45) % and (36.12±15.60)%, respectively, and the difference between two groups was significant (P〈0.01). The CD4^+/CD8^+ of them was (2. 64±0. 53) and (1.07±0. 45), respectively, and the difference between two groups was significant (P〈 0. 01). CONCLUSIONS The single test for CD3^ + lymphocyte production is not valuable for diagnosis. HCMV antigen can stimulate the CD8^+ lymphocyte proliferation, and there is significant difference between the HCMV unactive infection group and the HCMV active infection group. Thus, when HCMV active infection happened, the entire number and proportion for CD8^ + lymphocyte increase in peripheral blood resulting in the ratio of CD4^ + lymphocyte and CD8^ + lymphocyte decreasing or even reverse.
